Congratulations to our frailty unit at Ysbyty Glan Clwyd on its first birthday. It's been providing great care for our older patients and getting them home as quickly as possible. More than 620 discharges since last February. 🎉👏 #teamfrailty

Does any of my therapist followers work jointly with the ambulance services assessing frailty and falls?
Comprehensive geriatric assessments allow for identification of the multidimensional aspects of older patients care. Use of a CGA allows more accurate diagnosis as well as making a treatment plan that takes into account what matters most to those we treat! posters.geriemeurope.eu/post…
