We’ve covered the three easy to treat UTIs, and now on to the really f*!@in’ complicated one! Can we really afford to mess around with complicated CRE UTIs?

A multicenter, retrospective study(N:639)
The first study to compare clinical outcomes between patients who received cefepime & meropenem in a subset of patients with AmpC-ECK E bacteremia expressing baseline ceftriaxone resistance(MIC > 1 µg/mL)
🔺Among patients infected by ceftriaxone-resistant AmpC-E, carbapenems were used as definitive treatment in 30% (65/214), while cefepime was used for definitive treatment in 41% (88/214)
🔺After propensity score weighting, no statistically significant difference in 30-day mortality (HR, 1.08; 95% CI, 0.49–2.39; P= 0.848) was observed between patients infected by ceftriaxone-resistant AmpC-ECK E who received definitive treatment with meropenem or cefepime #IDXposts

Treating CRE is no joke, so why is IDSA joking around? A lot of these suggestions are a roll of the dice – probably not the way I would go if I was writing a gram negative guidance document!

There is no standardized treatment catastrophic brain injury (CBI), a fatal injury. Giving dopamine, DDAVP, vasopressin, levothyroxine, or steroids alone: ↑ odds of organ donation after CBI. DDAVP and steroids together have the highest odds of successful donation.

It is long past time clinical guidelines incorporate the humility of uncertainty & stop creating standards of care absent of reproducible prospective studies.

Fresh blog: Hot take on the VASSPR trial 🔥
🗜️Numerous correlational and target-trial-emulation studies published in the past five years suggested that early vasopressin initiation could cause huge mortality benefits.
🗜️ The VASSPR trial, a new multicenter RCT, shows that early initiation of vasopressin doesn't affect mortality or renal failure.
🗜️This is a great illustration of why we need RCTs. Sometimes a dozen retrospective & target-trial emulation studies are all consistently wrong.
🗜️Vasopressin can impair digital perfusion, which may be VERY problematic if you're using capillary refill time as a resuscitative target. (#1/2)
