Adjunctive targeted disruption of the Purkinje network improved outcomes after VT ablation in patients who also had evidence of VF. It is critical to review every episode of ICD therapy and plan accordingly.
A #JACCCEP study showed that in pts. w/ prior spontaneous polymorphic VT/VF, adjunctive Purkinje de-networking during VT #epAblation was associated with a substantial reduction in recurrent ventricular arrhythmias, w/ a modest increase in risk of LBBB.

In VT ablation, does scar architecture matter more than scar burden, and is noninducibility equally meaningful across substrates?
Pleased to share our editorial with @utedrow, discussing the intriguing findings of Pittorru et al. in @JACCJournals.

Substrate de-networking linked to greater freedom from VT/VF but more conduction system injury. Trigger-focused ablation may be considered first when PVC triggers are identifiable, reserving substrate modification for refractory/nontriggered VF jacc.org/doi/10.1016/j.jacep… #JACCCEP

Excited to share our new editorial comment, “New Energy, Old Lessons” discussing the evolving safety profile of pulsed field ablation. Now available at @JACCJournals. Grateful to Dr. @True_EP for the invitation and mentorship on this work.
Pleased to share our latest publication in @JCardioEP. We describe a preventable cause of elevated shock impedance during EV-ICD implantation and a simple strategy to help avoid it. #EPeeps #CIED @SoSunny14

Calcified LV thrombi can block catheter contact during VT ablation, limiting energy delivery & leading to recurrence.
