🫀⚖️🩸 📝🆓️⤵️ Position Paper @ESC_Journals 💯 doi:10.1002/ejhf.3782 t.me/medicinainternaconsoque…

AF burden is increasingly being looked at as an outcome for clinical trials owing to the known limitations of time to event recurrence endpoints. In the attached clinical trials workshop we critically examine the role of AF burden as an outcome for clinical trials of AF interventions, evaluating the evidence supporting AF burden as well as providing framework for an analytic strategy. We also delve into complementary metrics such as LEAF (longest episode of AF) and AF density. @CardioUBC @DilawriCVI @VCHResearch evidence.nejm.org/doi/full/1…

✍️Young-onset AF is linked to significant adverse events, especially with genetic or structural 🫀disease. ⚡ Catheter ablation outcomes remain favourable in young pts, but LGE may identify those at highest risk.

This video refers to a chaotic, ineffective heart rhythm where the ventricles shivers instead of pumping. On cardiopulmonary bypass (CPB), the goal is often to convert fine (low-amplitude) VF into coarser VF before delivering a shock, because coarse VF converts more reliably to a sustainable rhythm. Fine VF on CPB rarely converts with an immediate shock and the monitor often barely registers it. On full cardiopulmonary bypass support, briefly raise pump flow and hold it for a short period. This coarsens the VF waveform (increases amplitude), making defibrillation far more likely to succeed and restore a viable rhythm. Shock only after it thickens. (See videos for better understanding) A common miss is treating this like standard ACLS fine VF and shocking right away, or failing to recognize that the low-amplitude activity may not even appear on the monitor. The temporary flow increase is a simple, practical step that takes advantage of the full support already in place.

Very excited to see 8 years of work coming soon @HRS_O2Journal. The rationale for dopamine is to avoid alpha agonism at all cost in HFrEF. The majority of VT is mappable with anticipation, strategic positioning, and pharmacologic manipulation. Congrats @ZJameria @TakuroNishimu @gauravaupadhyay!
