Nine year old Dylan Cope died from sepsis. His GP had suspected appendicitis and sent him to hospital.
A paediatric nurse practitioner assessed him in A&E without reading the GP’s referral. His abdominal pain was attributed to flu and he was discharged without a documented examination by a doctor.
The coroner concluded that his death could have been avoided.
Dylan’s family have suffered an unimaginable loss. As a doctor working in general practice, what stands out to me is that his GP had recognised the possibility of appendicitis and referred him for further assessment. That concern needed to be heard and acted on.
When we refer a patient with a suspected serious illness, we need to know that the concern prompting that referral will be properly assessed. A different diagnosis may emerge, but the referral’s findings must be considered before a patient is sent home.
Dylan’s father told the inquest that he believed the practitioner assessing his son was a doctor. He also repeatedly raised concerns about appendicitis. Families deserve clarity about who is treating their child and to have their concerns heard.
This case raises serious questions about how referrals are handled and when an examination by a doctor must take place. Employers must ensure that supervision arrangements translate into the assessment a patient actually needs.
There is also a wider discussion about advanced practitioner roles. The BMA recognises their valuable contribution within healthcare teams. It calls for nationally agreed limits of practice and clear supervision requirements, with advanced practitioners supporting medical teams rather than replacing doctors on clinical rotas. Its position is that they should not independently diagnose or manage patients whose underlying condition has not yet been established.
I support those safeguards. Clear responsibilities protect patients and support staff to practise safely.
Dylan’s parents are asking for children referred to A&E by a GP with suspected serious illness to receive a documented, face to face examination by a senior doctor before discharge.
That is a call we should take seriously. They should not have had to lose their son to ask for it.

Today, there was a statement from the President of the United States that an energy ceasefire has been secured. This is good news for everyone – and it is important that it become reality.
Unfortunately, Ukrainian skies have been infested with Russian “shaheds” all day today. There have been glide bomb strikes and strikes on our energy infrastructure too. A power plant in Kharkiv was attacked this evening.
Overall, this past week alone, the Russians have carried out more than 200 strikes on our electricity generation and transmission facilities in Ukraine, and another 17 strikes this week targeted gas and oil facilities. They have killed more than 150 of our people, and more than 700 people have been wounded.
Ukraine has proposed many de-escalatory steps. Putin had one answer to all of them – “nyet.” We do hope that he will finally say something to America, Ukraine, and the entire world that will lead to a real and dignified peace.
Ukraine wants peace. The question is whether Russia does. The Armed Forces of Ukraine have received all the appropriate orders. We are awaiting clear details of the dialogue between America and Russia – and we expect Putin to follow through on what President Trump said.

The ruthless #circumcision doctors never ask themselves about the cumulative effect of sending home a nation of angry, wounded babies into the world, as if there could not possibly be consequences from the physical, psychological, and spiritual damage from this traumatic amputation.

The Observer’s closeness to Tony Blair & his institute - which has successfully specialised in persuading the Lab govt to go full tech bro - is a recurring theme. Remember this front page???

It will shock you to learn that the executive director of the Tony Blair Institute is endorsing the main editorial in today’s Observer’s: a manifesto for AI boosterism.
But that’s not the only thing going on here.

I’ll do a thread later but if you want to read Tom Watson’s WhatsApp to me defending his position on Palantir’s advisory board, read on. This was from before he took his current executive role last week. His defence? That he didn’t have an executive role..

NEW: NEW: Meet the real AI ‘rogue agent’: Lord Watson of Palantir.
I am sick to the teeth of this current AI hype cycle & its perversion of language. The only thing going ‘rogue’ right now are AI boosters, a gullible & compliant press & a Labour Lord

It’s pretty nuts. Tom Watson’s very first intervention in his new job at spytech firm Palantir is to write a letter to his former colleague @labourlewis.bsky.social
detailing all the times the firm has monitored his anti-Palantir activities

And this is *exactly* why we need independent journalism, folks. And the only hope of that is if people pay for it. This is not news, it's propaganda.

If you read the Guardian, support the Guardian, believe the Guardian should hold itself to the standards it expects of others, you need to ask how it could ‘employ’ journalists over years on these terms..& then simply dump them when the law finally catches up with them.

Last night, I moderated a panel after a screening of the movie which included Frances Haugen. She points out Sarah Wynn-Williams is the first Facebook whistleblower to be sued. "And because she can't speak out I will". Haugen says FB's "unique" pursuit of her means her allegations are "likely true"

It’s dishonest & embarrassing to pretend I don’t exist & that Banks didn’t sue me as a result of this work. It contributes to the ongoing silence that’s accompanied this story for a decade. But most of all, it insults the readers.
