The proposed addition of folic acid to flour would expose consumers across the wider population on an ongoing basis to pursue a benefit measured against that annual number of live births. We ask ministers to publish how they weighed that difference in scale, including the potential effects of long-term exposure on older people, before the requirement begins.   I recognise the benefit of adequate folic acid before conception and in early pregnancy in reducing neural tube defects.  I ask the government to compare mandatory flour fortification with a properly funded programme that helps women who may become pregnant obtain and take supplements in time. This comparison must address unplanned pregnancies: waiting until pregnancy is confirmed may be too late to prevent some neural tube defects.  Roughly 30-60% of the population has some variation of the MTHFR gene, which impairs their ability to convert synthetic folic acid into the active, usable form (L-5-MTHF). For these individuals, unconverted folic acid can build up in the bloodstream. This could represent a cancer and metabolic disease risk.  Given the present burden of cancer, I ask for a clear public account of how cancer-related research was assessed. NHS England recorded 354,820 new cancer diagnoses in England in 2023, 8,605 more than in 2022. Those figures do not show that folic acid causes cancer. They underscore the importance of examining any plausible concern before adding a further source of folic acid to widely eaten foods.  One randomised trial involving people with a previous colorectal adenoma found a higher occurrence of multiple adenomas during later follow-up among participants assigned 1 mg of folic acid daily. This dose and study population differ from the proposed flour policy. Please explain how the trial was assessed alongside the wider evidence when considering cancer risk.  I also ask you to examine the findings for postmenopausal women. In a study of 105 healthy postmenopausal women, researchers detected unmetabolised folic acid in 78% of fasting blood samples. Women with detectable unmetabolised folic acid had, on average, approximately 23% lower natural killer (NK) cell cytotoxicity in the laboratory test used by the researchers. NK cells help identify and destroy abnormal or infected cells. This was an observed association in one study; it did not establish that folic acid caused the difference or that the women developed cancer. It does, however, warrant an explicit assessment of immune measures and exposure in older women before the policy starts.
