Anytime someone claims that Australian medicine doesnt have a massive racism problem they should just be directed to r/ausjdocs for blatant demonstrations

Ouch. Each publication aimed at standardising airway management seems to only ever complicate the topic, when clarity matters most.
In this editorial, @dr_imranahmad & @doctimcook consider the PUMA tracheal extubation guidelines and ask - towards clarity or complexity? "Should we continue to develop guidelines tailored to specific clinical scenarios, considering factors such as location, speciality and patient characteristics? Or is it possible for a single, universal guideline to be applied globally, like the PUMA group envisages?" #anaesthesia @NicholasChrimes @UniversalAirway doi.org/10.1111/anae.70436

The planned regulation by the GMC of PAs will leave local employers to decide what exactly PAs will be doing, as with nursing associates. Another example here of why local employers can't be trusted:

🚨 Amos did find evidence of normal birth. Her own expert reviewers agreed the wording only for Amos to unilaterally change it prompting Kirkup to resign. Was Amos involved in a cover-up? Why? Who got to her? Another nail in the coffin of the Amos review: bbc.co.uk/news/articles/c3ey…

Yes but the anaesthetists who recognise they can secure their career & increase their pay by training & then supervising nurse practitioners in multiple theatres, rather than doing one list, and believe this can be done safely, won’t oppose it.

High inter-rater agreement only means better data & safer care if the info being agreed upon is clinically useful, which POGO is not. It pointlessly sub-classifies varying degrees of easy view POGO 1–100% (which can all be addressed w routine intubation) but can’t distinguish bw views that drastically alter the appropriate approach (CL 2b, 3a, 3b & 4 are all POGO 0%).

An incredible response from readers to my storh yesterday. A lot of people suggesting midwifery should go back to being a nurse entry role first with post-grad specialist training, or that it should be a 4 year degree. Thoughts?
🚨 INVESTIGATION: Britain's maternity crisis has deeper roots, extending into the universities trusted to train the next generation of midwives. A majority are still promoting a normal birth ideology while women are more complex than ever: READ: thetimes.com/article/b4fe112…

There is no particular reason UK F2s can’t similarly be trained more rapidly to become accomplished and independent specialists, @wesstreeting. UK medical training is some of the longest - read, least efficient - in the world. If you want to revolutionise NHS care, start here.
When you're attending let's you do your first independent 6-vessel angio as a PGY2🤙🏻@UAZNeurosurgery @WuyangYang1

Meanwhile the UK equivalent PGY2 is likely facing unemployment. NHS workforce planning has been a mess for decades, but the latest debacle is a scandal - of gross misspending of public money; of erosion of professional standards; & of preventable failings in care - which merits wider public scrutiny & outrage. @ShaunLintern

Why not just do a proper RSI and then start the TIVA infusion after?

Is ‘watch & wait’ an appropriate clinical approach 1.) in an era of rapidly rising cancer rates in young patients, and 2.) when access to diagnostic testing should - in theory - be easier than ever? Accept that this is a widely adopted approach in UK general practice, but wonder whether it’s still the right one?
Most of us have always done this when there is no clear dx. 1st appointment: ok to watch and wait. 2nd appointment: do something (investigate/prescribe). 3rd appointment: refer, ask colleagues for fresh eyes, try a different treatment, investigate further or all of the above.

Another adject failure of NHS workforce planning. More radio silence from @NavinaEvans et al. Where is the accountability & scrutiny for such a gross mismanagement of public funds? @ShaunLintern @wesstreeting medical.hee.nhs.uk/medical-t…

This is absolutely astonishing. Nurse-led units with virtually zero medical input. One has to stop and think for a second: Why? These people keep banging the MDT drum, and then go rogue and fly solo. We seriously need to recalibrate and infuse some sense into the situation.
🏴‍☠️ I’m not having a pop at nurses, but this nurse in particular is displaying the same delusions of grandeur as some Physician Assistants and has no clue about her scope of practice. ⭕️ She has no doctor or medical consultant to discuss cases with, only a NURSE consultant or a PARAMEDIC. ⭕️ She is managing unresponsive patients alone without even being an Advanced Nurse Practitioner, and is only a forensic nurse 💀. Do you think your professional registration with the NMC as a nurse covers such a big liability with no doctor around? ⭕️ She thinks ST3 doctors (registrars with MRCX qualification) are “three weeks old” compared to nurses.☠️ I have to admit, this is an advanced level of quackery and an unprecedented display of boastful behaviour from a nurse.
