This Comments and Controversies piece out on JCO argues that while debate continues around the adjuvant use of CDK4/6 inhibitors in hormone receptorāpositive/HER2ā early #BreastCancerādriven by trial heterogeneity, cost-effectiveness concerns, and methodological critiquesāthe evidence from randomized clinical trials supports their use in patients meeting the high-risk criteria established in registration studies. The authors emphasize that both abemaciclib and ribociclib demonstrate significant reductions in recurrence risk with a favorable quality-of-life profile, and caution that dismissing these agents outright would deny meaningful benefit to appropriate patients.
They advocate for refining patient selection through predictive biomarkers to improve cost-effectiveness, and call for a rigorous, unbiased reassessment of the metrics and endpoints used to evaluate (neo)adjuvant therapies in this disease setting.
