Adjuvant RT may finally be back in bladder cancer.
The phase III BART trial shows that modern pelvic IMRT after cystectomy significantly improves locoregional control in high-risk MIBC. 🚨

90% received perioperative chemotherapy

87.1% vs 76.0%

Most striking finding:
❌ No isolated pelvic recurrence after adjuvant RT.
Despite historical concerns, toxicity remained low with modern IG-IMRT:
⚠️ Grade ≥3 acute toxicity: <5%
⚠️ No major increase in severe late toxicity
This is important because pelvic failures after cystectomy are often devastating and rarely salvageable.
The study strongly reopens the discussion around integrating pelvic RT into multimodality care for high-risk bladder cancer, especially in the modern immunotherapy era.
Could adjuvant RT + IO become the next frontier in MIBC?

We learned a lot about muscle-invasive bladder cancer this past week. Together, these studies suggest that a strategy worth exploring is to further de-intensify surgical lymph node management to reduce surgery-related mortality, while using adjuvant radiation to treat nodes in the ~30% of patients found to have pathologic T3-4 disease.
1. SWOG 1011 (Lerner SP et al NEJM 2024) shows that a more extensive pelvic LN dissection causes more harm than good, even in clinical T3-4 disease. (>10% of patients in extended lymphadenectomy group died within 6 months of randomization.)
2. SWOG 1011 suggest that if we could better manage pelvic LNs in pts with pathologic T3-T4 disease, we might improve cancer control. (HR~2, but we can’t know if a patient is pathologic T3-T4 till after radical cystectomy due to clinical-pathologic discordance problem.)
3. Marcq et al (JCO 2024) from McGill U. suggests that RT to pelvic LN is effective (in intact setting). (Multiple RCTs from Zaghloul M et al suggests RT effective in post-op setting too (most recent in IJROBP 2024)).
4. BART Trial (Murthy V et al ASTRO 2024) from Tata Memorial Hospital suggests that adjuvant radiation after radical cystectomy does not appear to meaningfully increase late toxicity. (Small bowel obstruction rates, our greatest worry, were similar with and without adjuvant RT.)
