‼️ On 134 key questions in prostate cancer care, experts reached consensus on just 44.
What does that tell us about the evidence guiding treatment — and where do we need better answers?
📄 Management of Patients with High-risk Localised, Locally Advanced, and Biochemically Recurrent Prostate Cancer: Report from the 2026 Advanced Prostate Cancer Consensus Conference
🔗 sciencedirect.com/science/ar…
At #APCCC26, 144 international experts 🌐 tackled challenging clinical questions where high-level evidence remains limited.
Using a modified Delphi process, 133 panellists voted on 134 questions covering high-risk localised, locally advanced and biochemically recurrent prostate cancer, including PSA persistence, PSA recurrence and treatment-related side effects.
✅ Consensus (≥75% agreement) was reached on just 44 questions (32.8%), with strong consensus (≥90%) on only seven.
👉 Where experts disagree, the need for better evidence is clear. Areas without consensus highlight important priorities for future clinical trials.

🔬 Phase 3, 2×2 factorial (n=1173): abiraterone and/or prostate RT added to SOC (ADT ± docetaxel) in de novo mCSPC. Median follow-up 7.7 y.
➡️ Abiraterone: OS HR 0.75 (0.65–0.87); p=0.0001
➡️ ADT + docetaxel ± abiraterone: median OS 4.4 vs 6.2 y; 8-year OS 27.1% vs 40.3%
➡️ High-volume: 8-year OS 20% vs 38%; interaction by metastatic extent p=0.56
✅ Major OS benefit of adding abiraterone to SOC, irrespective of metastatic extent; triplet associated with almost twice as many long-term survivors in some subgroups.

Can three routine blood tests improve risk stratification in advanced prostate cancer?
ascopubs.org/doi/10.1200/CCI…
Development and External Validation of a Machine Learning-Derived Risk Stratification Score for Metastatic Castration-Resistant Prostate Cancer
PROGRESS is a machine learning-informed prognostic score developed to improve risk stratification in metastatic castration-resistant #ProstateCancer (mCRPC).
➡️Combining three routine laboratory measures—PSA, alkaline phosphatase, and AST—the tool consistently distinguished patients with different overall survival outcomes across independent validation cohorts.
➡️ Its simplicity and reproducibility may support clinical decision-making and trial enrichment, although prospective real-world validation remains necessary before widespread implementation in clinical practice.

🧬 Could a tumor’s androgen receptor mutation determine whether a novel prostate cancer therapy works?
ascopubs.org/doi/10.1200/JCO…
Phase I/II Study of Bavdegalutamide, a Proteolysis-Targeting Chimera Androgen Receptor Degrader, in Metastatic Castration-Resistant Prostate Cancer
Bavdegalutamide (ARV-110), an oral PROTAC androgen receptor degrader, demonstrated manageable safety and promising activity in metastatic castration-resistant #ProstateCancer (mCRPC).
➡️In this phase I/II study, responses were particularly notable in tumors harboring AR T878/H875 mutations, with a 60% PSA50 response rate and median radiographic progression-free survival of 11.1 months.
✅These findings highlight the potential of molecularly guided treatment and targeted protein degradation in advanced prostate cancer, warranting further clinical investigation.

Can repeated metastasis-directed radiotherapy ☢️delay ADT in oligorecurrent prostate cancer—and which patients benefit most?
thegreenjournal.com/article/…
Repeated metastasis-directed radiotherapy in patients with oligorecurrent prostate cancer: outcomes and risk stratification from a large multinational PSMA PET-guided cohort
Repeated metastasis-directed radiotherapy (MDRT) may help selected patients with oligorecurrent #ProstateCancer delay androgen deprivation therapy (ADT).
➡️ In this 177-patient multinational cohort, 50.2% remained ADT-free two years after their second MDRT cycle.
➡️Longer intervals between treatments and fewer new metastases independently predicted longer ADT-free survival. While promising, the absence of a comparator group highlights the need for prospective studies to clarify treatment benefit and optimise patient selection.

🔬 Can genomic classifiers help us personalise salvage treatment for recurrent prostate cancer after radiotherapy?
ascopubs.org/doi/10.1200/JCO…
Dose-Escalated Focal Salvage High-Dose-Rate Brachytherapy and Genomic Classifier Risk Stratification in Radiorecurrent Prostate Cancer: The F-SHARP Phase I/II Clinical Trial
The F-SHARP phase I/II trial evaluated dose-escalated focal salvage high-dose-rate brachytherapy (DE-fsHDR) for intraprostatic radiorecurrence following prostate radiotherapy☢️
➡️Among 62 patients, treatment demonstrated a favourable safety profile, with no acute grade 3 toxicities and 3.2% late grade 3 toxicity.
✅Three-year metastasis-free survival reached 85.9%. Higher genomic classifier scores correlated with poorer outcomes, highlighting their potential to identify patients who may benefit from treatment intensification. Further validation is needed. #ProstateCancer

Mathematical Biomarkers of Adaptive Therapy Outcomes in Prostate Cancer
This modeling study of 53 patients across two clinical trials found that mathematical biomarkers derived from initial PSA dynamics predicted progression and overall survival in #ProstateCancer patients receiving adaptive hormone therapy.
➡️Unlike conventional PSA 🧪measures, these mechanism-based biomarkers 🎯captured tumor sensitivity and resistance dynamics, offering a promising, noninvasive approach to personalizing treatment schedules.
➡️Although retrospective and based on small cohorts, the findings support further prospective validation to advance individualized, evolution-informed cancer care.

➡️ Although combination therapy achieved a 73% PSA response rate at 12 months, superiority over monotherapy was not established.
➡️ Grade ≥3 adverse events were more frequent with combination treatment, including two fatal myeloid neoplasms.
✅ Tumor analyses revealed androgen receptor inhibition altered gene expression without impairing functional homologous recombination, highlighting challenges in identifying patients most likely to benefit.

Are we underestimating aggressive prostate cancer by missing its most important histological warning signs?
nature.com/articles/s41585-0…
Cribiform and intraductal carcinoma in prostate cancer-implications for risk stratification and therapeutic decision making (REVIEW)
Intraductal carcinoma of the prostate (IDC) 🔬 and invasive cribriform carcinoma are aggressive histological patterns associated with adverse pathology, metastasis, and prostate cancer mortality.
➡️ Frequently underdetected on biopsy, they exhibit PTEN loss, genomic instability, DNA-repair alterations, and immunologically cold tumour microenvironments.
➡️ Although these features suggest potential biomarker-directed therapies, evidence remains insufficient.
✅ Standardized reporting, improved detection, and prospective studies are essential to guide risk stratification and treatment decisions. #ProstateCancer
