Even with sensitivity of 27% for cytology (most series now have better yield), it is more cost-effective to use thoracentesis as 1st investigation #MedEd
Rather than focusing on more invasive biopsies, effort should be spent on improving your unit's cytology yield (the higher; the more cost-effective).
Have specialist cytologist = 1st step
Have specialist cytologist with pleural interests who read all pleural samples = ideal

Key points 1:
Treatment success is NOT CXR clearance.
If infection settles, residual pleural opacities will resolve with time - don't send to VATS just for CXR clearance.
