Excited for the interest in our biomarker work from the urologic oncology community @VincentWenxinXu @PavlosMsaouel @AlexSherryMD @kieko_hara @d_shapiro1 @MDAndersonnews
🏆 Congratulations to @ChadTangMD et al. on receiving our April IMPACT Award!
This month's winning paper stands out for its contribution to understanding how we can better predict outcomes for patients with Oligometastatic Clear-cell Renal Cell Carcinoma — an important step forward for personalised care.
Well deserved. 👏

Replying to @GiulioFrancoli1
@GiulioFrancoli1 at #estro2026 presents early results of the randomized PERSIAN trial. Results are early showing excellent PSA response in both arms (mdt slightly better) but definitely more to come.

New @EurUrol! Pooled trials @PeterMacCC / @UTMDAnderson shows adding Pembrolizumab to MDT for oligometastatic #kidneycancer ⬆️ PFS (28 vs 17 mo; HR 0.57), esp. after prior therapy - shorturl.at/WWvBg - Correlative data confirm MDT+ICI-evoked immune activation #Kcsm #RadOnc

Excited to review our paper on KIM-1 and ctDNA in oligomet RCC and the K-COMPASS prognostic model published this wk in @EuropeanUrology. Great collaboration between @UTMDAnderson and @DanaFarber. Specifically @VincentWenxinXu and @DrChoueiri

This is how we finally start harnessing the untapped for decades power of *prognostic* modeling (not “predictive”) for biomarker development. Onwards! 🔬@ChadTangMD @VincentWenxinXu @AlexSherryMD @OncHahn @AmandaNizamMD @KidneyCancer @ebludmir @PGrivasMDPhD
The K-COMPASS study by @ChadTangMD shows how ctDNA and KIM-1 are prognostic and can be used to estimate treatment-free survival in patients with oligometastatic RCC #ASCOGU26 @PavlosMsaouel

At #GU26 a tremendous overview of optimal trial design by @PavlosMsaouel. Pavlos has repeatedly shown that even in very rare cancers, optimally designed studies can rapidly led to practice changing findings. A visionary in our field.

Extremely excited to share our latest article published @TheLancetOncol a few hours ago: authors.elsevier.com/a/1mYT6….
The WOLVERINE individual patient meta-analysis was an international collaboration and part of X-MET collaboration. Goal was to evaluate MDT in oligomet prostate ca.

1/5 How reproducible are phase III oncology trials?
📊 With @AlexSherryMD @ErikVanZwet @ebludmir et al. We analyzed 632 RCTs (~496,000 pts). The data offer both reassurance and caution for clinicians and researchers. pubmed.ncbi.nlm.nih.gov/4062… 👇#MsaouelLab

🐺Dr @ChadTangMD presenting Abs15 "World-wide oligometastatic prostate cancer (omPC)meta-analysis leveraging individual patient data (IPD) from randomized trials (WOLVERINE): An analysis from the X-MET collaboration"
5⃣ RCTs omPC: MDT+SOC vs. SOC
[STOMP, ORIOLE, ARTO, SABR-COMET, EXTEND]
n = 472 (248 MDT+SOC, 224 SOC,)
median follow-up = 41 mo
- CSPC (58%)
- treated with ADT (74%) +/- ARPI (54%)
- treated primary prostate (82%).
✅PFS 0.45 (0.35-0.58), P<0.0001
✅rPFS 0.59 (0.46-0.76), P<0.0001
✅CRFS 0.58 (0.37-0.91), P=0.020
✅OS 0.64 (0.40-1.01), P=0.057
PFS benefit across subgroups [castration, prior tx, staging, ADT/ARPI] (all HR < 0.52, all P < 0.05)
🩺Clinical implications:
- WOLVERINE demonstrated significant benefit of MDT for rPFS & CRFS (trend OS)
- Several ongoing studies to evaluate the role of MDT 👇

Proud to present the results of the EXTEND breast basket led by #JayReddy from @MDAndersonNews in the @IJROBP . This is a randomized trial of oligometastatic breast cancer patients randomized to +/- MDT. Unfortunately not all trials were positive.
