6 years ago, I had the chance to publish a paper that would end up changing my academic life It was a review article exploring the surprising activity of certain ADCs in “HER2-low” breast cancer, a newly emerging entity whose relevance was still uncertain. We tried to connect the dots, asking if HER2-low was a subtype vs just a target, wondering whether this concept could one day translate into meaningful benefits for patients. 6 years later, that article has reached 1,000 citations. My career has gifted me more than I could ever have imagined. More importantly, what was once an intriguing scientific observation has become a clinical reality: countless patients with HER2-low breast cancer now live longer thanks to T-DXd. A reminder of how far science can take us in just a few years. Grateful to everyone who has been part of this journey! ascopubs.org/doi/10.1200/JCO…

Thousands of clinical trials are looking for patients. Epic AI helps match trials with the right patients, giving them a new option for treatment. Since this spring, more than 60 organizations have started using it to automatically check study criteria against what’s already in the chart. Study teams can quickly see who may be a fit and invite them right in MyChart.

Question for the ESMO breast cancer crowd: what would be a good study to present to heme/onc fellow academic half day that will change standard of care significantly ? @PTarantinoMD @stolaney1 and others!

Primary results from BRIDGET ph2 trial presented by @drsarahsam and published in @ESMO_Open. Adding tucatinib to HP after an isolated intracranial PD led to impressively long PFS (59% free from PD or death at 18mo!!). Reasonable strategy from next Monday in TKI-naive patients.

Presented at #ESMO26: In patients with 𝘌𝘎𝘍𝘙-mutated and MET-altered non–small-cell lung cancer that had progressed during osimertinib therapy, osimertinib plus savolitinib (a MET tyrosine kinase inhibitor) led to longer survival than platinum–pemetrexed. Tucatinib added to HP is a nice option for 1L low volume brain only disease after local therapy. Particularly in patients with no extracranial disease where the toxicity of ADC is not desired. #ESMO26

The SWITCH trial adds important prospective data on TOPO1 ADC sequencing. Among TOPO1-ADC pretreated pts with MBC, barely any activity was observed with a second TOPO1 ADC, with <10% ORR and <3 mo PFS, despite switching target. Payload cross-resistance dominates. #ESMOAmbassadors

Switching ADC target does not work. Switching payload does. Among 60 TOPO1-ADC-pretreated patients with MBC, using a microtubule (MMAE) ADC led to 30% ORR and DoR of 8.8 mo. #ESMO26 #ESMOAmbassadors

Significant OS benefit with Sac-TMT vs chemo for highly pretreated mTNBC, consistent with results from ASCENT (SG). Multiple global phase 3 trials ongoing with this Trop2 ADC, which will likely expand its role beyond 🇨🇳 (where it is already approved) #ESMO26 #ESMOAmbassadors

Presented at #ESMO26: Among patients with PD-L1–negative advanced nonsquamous non–small-cell lung cancer, progression-free survival was longer with sacituzumab tirumotecan plus pembrolizumab than with chemotherapy plus pembrolizumab (15.0 vs. 5.7 months). TROPION-Breast02, brain mets subgroup at ESMO 2026. First-line Dato-DXd vs chemo in mTNBC, n=61. CNS PFS 9.5 vs 2.6 mo, HR 0.27. Intracranial ORR 62.5% vs 37.5%. Longest intracranial response about 27 mo. In TNBC, brain mets come early and we usually treat them locally first. This makes me ask whether a patient with small asymptomatic lesions could start Dato-DXd and defer /combine SRS Not ready for clinic. This is a post hoc subgroup, n=30 with untreated brain mets. And even on Dato-DXd, median OS was 11.6 mo with brain mets vs 23.7 mo without. The CNS benefit is real; the prognosis is still poor. The answer needs a dedicated CNS trial. #bcsm #ESMO26 @myESMO

For the final act of CheckMate 8HW, we finally have the significant OS data the doubters have been asking for. What happens when you have a great treatment and an even better one for a study (and almost everyone crosses over)? You have to wait a really, really long time for OS. Also, some thoughts on splitting the alpha I won’t get into here. Nivo/ipi vs nivo alone vs chemo, MSI-H/dMMR mCRC. Final OS analysis. Response rate 72% with nivo/ipi, 58% with nivo alone.
Median OS still not reached.
OS HR 0.66 for nivo/ipi over nivo, statistically significant.
77% of nivo/ipi patients alive at 5 years. Three out of four patients alive at five years. In metastatic colorectal cancer. 🤯 Not even going to talk about the chemo arm. It's clearly worse, and there is never a time to give chemo in MSI-H unless someone has extreme, active autoimmune disease. “Debulking” is absolute nonsense. Bottom line: the doubters got their OS. Nivo/ipi is the first-line standard for MSI-H mCRC. Next steps: how do we decide length of therapy, and how do we manage oligometastatic progression? #ESMO26

#ESMO26 | TALENT Neoadjuvant T-DXd showed very low pCR rates in HR+/HER2-low early breast cancer: - T-DXd: 7% - T-DXd + anastrozole: 3% - T-DXd → chemotherapy: 9% We should remember that pCR is a limited surrogate endpoint in HR+ disease. However, these results do not support T-DXd as an alternative to standard neoadjuvant chemotherapy. Moreover, given the heterogeneity of HER2-low disease, I find it unsurprising that the success of T-DXd in the metastatic setting does not translate into similar outcomes in early-stage disease, despite the lower tumor burden.
