Great debate at #AUA26 #SMU2026:is cystectomy still mandatory after cCR following ev/p
@JanetKukreja >1 in 3 cCR patients have residual cancer at cystectomy our restaging tools (cystoscopy, cytology, imaging, TURBT) all fail
@benjaminpradere ctDNA + utDNA together may let us safely spare the bladder

One of the major limitations of intravesical therapy is short drug exposure time. In this new study, the gemcitabine intravesical system (Gem IDRS) demonstrated sustained tissue levels of active metabolites for ≥96 hours across all bladder layers, compared with rapid decline after standard instillation.
Improving drug delivery may be as important as the drug itself. @BenjaminPradere @IBCG_BladderCA @BladderCancerUS @USC_Urology @urotoday @EurUrolFocus @EUplatinum sciencedirect.com/science/ar…

Agree 💯 Tom.
We need to help our patients and deliver a unified message: the OS advantage seen with EV+P
relies on consolidation with Radical
Cystectomy + PLND
(not observation, not TMT)

Live podcast #EAU26 @MichvdHeijden - the original source of ‘EVP 1st, ask questions later’ describes a new world of short periods on systemic therapy for MIBC followed by decision making uncertainty for the cCRs . @UroDocAsh & myself advocates for sticking to the protocol (EVPX9 and surgery) , but @brian_rini @shilpaonc think other patterns will emerge. @urotoday

Several talks about the role of #ctDNA & #utDNA at #GU26 with key insights from RETAIN, NIAGARA, TOMBOLA & SunRISe-4 and how they could be implemented in bladder-sparing approaches:
🧬 ctDNA is a strong systemic prognostic marker
✅Post-NAT ctDNA positivity → ~10x higher risk of inferior MFS
✅Persistent ctDNA after NAC —> very poor outcomes
✅ctDNA clearance correlates with better survival
🚫 ctDNA does not reliably capture local disease or recurrence 👉💧utDNA very promising:
✅about 80% utDNA+ at baseline
✅Clearance at week 12 associated with pCR
✅NPV ~70–76%, PPV ~84–88%

I'm especially excited to once again lead the "Common Problems and Controversies in Bladder Cancer: Rapid-fire Debates." Friday 13 March 10:45 - 12:15
We’ve designed this session to cut through the noise - no long lectures - just leading experts, opposing viewpoints, and the direct clinical insights you need for daily practice. It remains one of the most anticipated highlights of the meeting for a reason!

Jeremy Teoh presents a tour de force on how to optimize TURBT in management of patients with bladder cancer including techniques such as EBRT and Robotic TUR.
@jteoh_hk #GU26
@IBCG_BladderCA
It’s disappointing to me that TURBT continues to be dismissed as a “low-priority” procedure in so many training programs.
This single step defines the entire course for a bladder cancer patient. Accurate diagnosis, precise staging, and the choice of therapy all hinge on a high quality TURBT.
And the “advanced genomic analysis and molecular profiling” these same programs love to promote? It all begins with adequate, high-quality tissue obtained through a TURBT.
Yet incomplete resections leave behind residual tumor in up to 78% of cases at restaging, leading to higher recurrence rates and unnecessary progression.
If we are serious about improving outcomes in NMIBC - and even MIBC - we must treat TURBT with the respect it deserves: structured, deliberate training curricula, hands-on attending involvement, and real performance metrics.
This is why the @IBCG_BladderCA will make this one of the focus topics of our retreat this year, #IBCG26
