While employees ultimately bear the cost of employer-sponsored health insurance (lower wages!), employers are disengaged buyers for their employees. And every player in the healthcare sector loves this lack of attention: hospitals, physicians, insurers, pharma, benefits consultants, etc. The higher the spend, the more they win. A few purchasers are doing it differently (e.g., @Walmart , @CalPERS), but it's a slog, and the entrenched interests push back hard. Employers should act like smart buyers: choose providers worth paying for and make it easy for their workers to use them. Read my new @statnews piece: statnews.com/2026/10/29/heal…

A $1 trillion Medicaid “cut” sounds dramatic until you notice Medicaid spending never actually falls. The number reflects a reduction from projected baseline spending, much of which had risen sharply. That distinction matters. My new @Forbes piece: forbes.com/sites/anthonylosa…

A look at how/why Wisconsin health care costs so much. Research from @EconTony
How much difference can crossing a state line make for health care costs? A new report shared exclusively with The Center Square found Wisconsin hospitals receive 25.6% more than Illinois hospitals for the same services. Even within Advocate Health, Wisconsin hospitals received 23% more for identical services than the company's Illinois facilities. Researchers say consolidation and reduced competition in Wisconsin's hospital market could be helping drive the difference. Read more: thecentersquare.com/wisconsi…

Here's a fun one: Majors, stratified by highest and lowest lifetime earnings! There's one major that tends to earn less than people who don't earn any college degree.

Many are calling for health care price controls. In @nytopinion, Lorens Helmchen and I argue that price controls rest on a heroic assumption: regulators can know the right price and keep it from being shaped by the industries regulated. The answer isn’t price by decree. nytimes.com/2026/07/20/opini…

The editors of the @NEJM recently invoked Stalin's infamous agronomist Trofim Lysenko to warn of where the administration's proposed "political review" of NIH grants will lead. 🧵

And it misses the real danger. The rule builds a permanent mechanism the next administration inherits, and can aim wherever it likes. Both left and right should distrust it.

Maybe you've seen the PhRMA ad with the tall hospital executive named "Mark" telling staff to "markup" drug prices. But what's the program behind the gag? My new @Forbes piece on 340B: how a tiny 1992 safety-net program became an $81B opaque revenue mechanism most Americans have never heard of. forbes.com/sites/anthonylosa…
