Barack Obama’s recent riffs about President Trump (“He’s got the White House looking like Saddam Hussein opened a Cheesecake Factory!”) and Trump’s rambling response (“I passed three cognitive tests. I’m the only President that agreed to take them. And they’re hard, you know? Obama—Obama couldn’t have passed it.”) underscored the split-screen reality of the midterm elections, Susan B. Glasser writes. “Two Presidents, two Americas. For ten endless years, our politics has been all about the vast gap that separates these two men, and the near-impossible task of trying to understand how a country that elected Obama twice could, so soon after, do the same for Trump. Watching them both on Wednesday night, toggling between screens and alternate universes as they shot barbs at each other, was a reminder of how very long we’ve been stuck in this loop. “What’s different this time is a sense—and, until there are votes counted and races called, it’s just a sense—that real-world events have intervened to make this less of an intractably divided country and more of a place where at least some shared truths are still possible.” Glasser reflects on Obama, Trump, and the midterms that may see the country bridging its divides: newyorkermag.visitlink.me/62…

PANKU-Lung01 at #ESMO26: promising phase III results for EGFR×HER3 bispecific ADC iza-bren after EGFR TKI. PFS: 11.1 vs 5.7 mo (HR 0.40). ORR: 60.2% vs 34.5%. OS immature. China-only trial; global validation and optimal sequencing remain important. #LungCancer

#ESMO26 The brilliant Dr. @marinagarassino with biomarker analysis from the phase III KEYNOTE 671 study of perioperative pembrolizumab for resectable NSCLC showing association of T-cell inflamed gene expression profile and TMB with efficacy. Benefit across subgroups; inflamed profile associated with better prognosis. #ESMOAmbassadors

#ESMO26 Dr. @fifimcdrmh presents the SARON trial. Phase II of consolidation radiotherapy vs systemic therapy in pts with driver negative oligometastatic NSCLC after 6-8w of control with systemic therapy. Improvement in PFS with radiation 13.9 vs 8.9m but no difference in OS.

🎯 #ESMO26 | Excellent discussion by @benlevylungdoc on SARON and PERSEE! Two trials, two important lessons for advanced NSCLC. 🔹 SARON: Consolidative radiotherapy improved PFS and local disease control in oligo metastatic NSCLC, but no OS benefit (HR 0.99). 🔹 PERSEE: Pembrolizumab + chemo met its primary endpoint PFS but not OS though numerically better (not powered for OS) in PD-L1 ≥50% NSCLC. Key messages from @benlevylungdoc : • Oligometastatic disease needs better biological definition, beyond the number of lesions. • Treatment intensification should be guided by better patient selection. • More treatment does not automatically mean better outcomes! - Lower threshold to add chemo to pembro in PD-L1 >50% 👏 A thoughtful discussion with practical implications for our clinics. #ESMOAmbassador #LungCancer #NSCLC

‘New’ COCOON: Reducing adverse events with Amivantamab/Lazertinib 👉 Simplified skin care regimen (x6m oral tetracycline, ceramide cream, oral zinc) 👉 Step up dosing: Lazer x4wks BEFORE Ami starts ✅ Combined approach reduces all grade dermatological adverse events (HR 0.31) @ 3m ✅ No Gr2+ cutaneous adverse events @ 4m ✅ Gr2+ scalp AE just 3% 🤔 I have no problem starting Lazer 1 month before Ami, if it makes combination much more tolerable. (With FLAURA2 we already start Osimertinib before chemo, whilst we wait for chemotherapy unit slot) 🤔 Need PFS data to make sure new COCOON does not impact on efficacy, but I doubt it will 🤔 Pragmatic way make this regimen easier, which is needed because it is challenging #ESMO26 #LCSM

“Prespecified descriptive OS analysis.” Those words should give us pause. Calling this a “positive OS result” in a phase 3 trial manuscript is misleading when OS was outside the formal testing strategy. The results are encouraging, but an OS benefit has not been formally demonstrated in this analysis. I’m surprised this framing passed journal review. Oncologists and patients shouldn’t need to close-read a manuscript like English professors to understand whether a trial formally demonstrated an OS benefit. Put the bottom line up front. @Timothee_MD @oncology_bg @SusanGKomen @BCRFcure @OncoAlert @Annals_Oncology @myESMO @ASCO @AACR @SABCSSanAntonio

🫁 Can early ctDNA clearance tell us which patients with EGFR-mutant NSCLC can avoid chemotherapy? FLAURA2 suggests not yet. At ESMO 2026, serial plasma analysis from FLAURA2 showed remarkably similar EGFR mutation clearance with osimertinib + platinum-pemetrexed and osimertinib alone. By week 2, 64% of patients in both groups had cleared detectable plasma EGFR mutations. By week 6, clearance reached 88% vs 89%. Early clearance was prognostic, but it was not predictive. The PFS and OS benefits of osimertinib + chemotherapy were seen whether EGFR ctDNA cleared early or remained detectable. That matters clinically. A rapid molecular response cannot currently be used to identify patients who can safely de-escalate to osimertinib alone. The next step may require ultrasensitive multi-marker ctDNA assays capable of detecting molecular residual disease beyond simple EGFR mutation clearance. oncodaily.com/oncolibrary/lu… @AstraZeneca #OncoDaily #OncoDailyLung #ESMO2026 #FLAURA2 #Osimertinib #EGFR #ctDNA #NSCLC
