Question: Is the actions of the Independent Office for Police Conduct (IOPC) incompetence, a cover-up or systemic failure

I’m pleased to share with you some important correspondence sent to @TheFCA CEO Nikhil Rathi by @johnmcdonnellMP in his capacity as Chair of the @appgonifandffs.
The correspondence was in the form of an open letter, hence our being able to share it with you.
The letter is necessarily detailed and long, but a fair summary would be as follows:
The letter challenges the FCA’s handling of whistleblowers and calls for greater independence, transparency and accountability following the death by suicide of whistleblower Simon Andriesz.
There are three important topics covered:
1. Independence and scope of the Paterson Review
John McDonnell MP questions the appointment of FCA Non-Executive Director Lea Paterson to review the FCA’s handling of Simon Andriesz’s case. He argues that Nikhil Rathi’s evidence to the Treasury Committee on 13th October was contradictory and failed to unambiguously establish whether the review is confined to the FCA’s communications with Mr. Andriesz or extends to wider whistleblowing practices. He requests clear, published terms of reference, clarification of who controls the review’s scope and reconsideration of appointing an independent external reviewer. He also seeks confirmation that the report will be published in full with only redactions as necessary.
2. FCA participation in the APPG’s summit about protecting whistleblowers from retaliation
John McDonnell MP criticises the FCA’s decision not to provide an executive-level participant at the APPG whistleblowing summit on Monday 2nd November, offering only an observer. He argues that discussing general principles of whistleblower protection would not compromise the Paterson Review and urges the FCA to participate meaningfully and engage directly with whistleblowers. By not doing so the FCA misses a unique opportunity to learn through engagement; not a good look when the FCA is undertaking a lessons-learned review.
3. Independent survey of whistleblowers
The letter strongly challenges the credibility of the FCA’s 2022 whistleblower survey, which received only 21 complete responses from 68 selected individuals, out of 1,041 reports received during the relevant period. FOI disclosures show the exercise was conducted in a purposive and seemingly contrived manner. For example, only cases selected by the FCA’s whistleblowing case handlers were included. John McDonnell MP calls for an independently administered survey of every individual who has contacted the FCA about whistleblowing since January 2022, with the methodology, questionnaire and results published.
The letter calls for substantive answers to important questions covering whistleblower protection, regulatory responsibilities and historical FCA practices. It requests an urgent response on summit attendance and a comprehensive reply to the other issues raised within 14 days, emphasising that credible scrutiny, demonstrable accountability and a culture of openness are essential to restoring confidence in the FCA.
The full open letter is available for you here:
appgifffs.org/wp-content/upl…
We believe this is a legitimate and necessary challenge of @TheFCA's handling of whistleblowers, particularly following the death by suicide of whistleblower @SAndriesz.
And note that there have been serious concerns in Parliament and in particular by @CommonsTreasury about how @TheFCA deals with whistleblowers going back to at least 2016 - see here, by Lord Tyrie when he was Chair of @CommonsTreasury.
Also if you would like to express interest in taking one of the few remaining complimentary places available at the APPG summit in Parliament on Monday 2nd November, which is all about protecting whistleblowers from retaliation and dedicated to the memory of @SAndriesz, please see here:
docs.google.com/forms/d/e/1F…
Thank you for taking time to read what we believe to be an important message; we hope you agree.
Please share your thoughts and repost as you wish - we need to get this message 'out there.'

HE WARNED THE BANK. THE BANK COLLAPSED. YOU PAID THE BILL.
Paul Moore (@Paul_R_Moore) was Head of Group Regulatory Risk at Halifax Bank of Scotland. His literal job was to spot danger. In 2004, he spotted it. He told the board that HBOS was lending recklessly, growing too fast, and building a financial time bomb. They thanked him by sacking him.
KPMG was brought in to investigate his concerns. KPMG concluded everything was fine. The Financial Services Authority @TheFCA whose deputy chair was HBOS chief executive James Crosby, agreed. Nothing to see here.
Four years later, HBOS collapsed. Lloyds (@LloydsBank / @LBGplc) was pushed into a shotgun merger to absorb the wreckage, then needed a £20.5 billion taxpayer bailout to survive it. James Crosby eventually handed back his knighthood and forfeited part of his £25 million pension. He was not prosecuted. Nobody was.
Paul Moore went to the Treasury Select Committee in 2009 and laid out exactly what had happened. It caused, as one journalist put it, shockwaves through the British establishment. It also caused addiction, depression, and suicidal episodes for Moore himself.
He spent years warning anyone who would listen that the system was broken. He wrote a book called Crash Bank Wallop. He died in September 2020.
The men who built the disaster got pensions. The man who tried to stop it got destroyed.
If you think whistleblowers are protected in this country, Paul Moore's life is your answer.

THIS IS THE NEXT BIG UK SCANDAL
The Hidden Credit Line scandal has run since the 2000s.
NatWest (@NatWestGroup), Royal Bank of Scotland (@RBS_Help) and Ulster Bank (@UlsterBankNI) sold business owners loans with an interest rate swap attached.
It was marketed as protection if rates rose. It actually created hidden exposure against the customer's own assets that nobody clearly explained. When rates fell after 2008, banks used that exposure to call in debts, strip assets and push profitable businesses into insolvency.
Steve Middleton (@stevemiddi1) and Mark Wright, a former RBS manager, built BankConfidential (@BankConfidenti1) to collect the evidence.
Mark Wright put in about £60,000 of his own pension. Neither made a penny and both lost their careers for telling the truth.
In November 2022 they met the FCA (@TheFCA) with Lord Prem Sikka (@premnsikka) and derivatives expert Ian Tyler.
James Hurley (@jameshurley) of @thetimes had already written 4 articles calling it financial and accounting fraud. The FCA let NatWest review itself and concluded it was not technically criminal fraud.
BankConfidential published its full report in November 2025. On 14 April 2026 it prompted a Westminster Hall debate led by John McDonnell (@johnmcdonnellMP), chair of the APPG on Investment Fraud and Fairer Financial Services (@appgonifandffs). MPs asked for a judge led statutory inquiry under the Inquiries Act 2005. Six reviews had already produced no justice.
Ian Byrne (@IanByrneMP) said this scandal belongs alongside Hillsborough, Post Office Horizon and infected blood. He also noted the FCA is a private company limited by guarantee with immunity from civil liability.
Andrew Candy was hit with an undisclosed £70,000 break cost. He lost his business and family home and spent 17 years seeking answers.
Economic Secretary Lucy Rigby (@LucyRigby) replied that the FCA is independent and ministers should be careful about interfering.
The regulator failed for over a decade, and its independence is now the reason given for not investigating that failure.
Nobody has gone to prison... yet.

We demand the truth and justice for Simon.
Not a lessons learned review into how @TheFCA could 'do better next time'.
There should not be a next time, a next Simon...
We are not dealing with indifference, we are not dealing with negligence we are dealing with wilful neglect, intentional harm.
Like or dislike what I am saying but if a director in a building company allowed scaffolding without safety barriers to be used to save costs for his employer, leading to a death they would face a legal challenge of corporate manslaughter.
It would not lead to a 'lessons leaned' review of should we put safety barriers up next time. Its not about stopping the next death its accountability for the one that just happened. No matter what you think now Mr Rathi, you do not walk away from this one.
Was it your job @Meg_HillierMP to soft soap Rathi whilst he smugly accepted no responsibility for his personal negligence?
Where was the @CommonsTreasury in challenging anything he said, is there a single member of the TSC with a backbone... or any integrity beyond their own pay packet and promotion prospects.
It was beyond embarrassing & IMHO you should be re-naming yourselves as the Treasury "Un-Selective" Committee prepared to swallow any lies you're given for self promotion.
Your failure to support and promote the interests of a significant whistle-blower, who ended his life largely due to the failure of @TheFCA whilst he named naming Rathi in particular was dsigracefuk.
Your performance on Tuesday @Meg_HillierMP proves you are not fit for that role, step down and stop letting real people down.

Accountants/IPs enabled HBOS Reading, hid the theft of £1 billion.
The accountants signed off 'rigged' accounts in @NatWestGroup & @LloydsBank in 2008/09/10 to hide the missing hundreds of billions (not tens) until that could be stolen back from SME customers.
I'm dealing with a number of cases at present where forged documents have led to insolvencies, administrations and bankruptcies. Forgeries done not by those in financial hardship but dome by the accountants & IPs, to create financial hardship they can profit from.
Not even good forgeries like @natwest e-disclosure produce with their extracted and doctored PDFs but old fashioned composites, overlaid photocopies.
They are at the heart of financial corruption in the UK as @premnsikka so rightly called it with use of their 'Fiddle Factories', creating fake documents.
Time for a public inquiry into the accounting & insolvency 'professions' and proper independent regulation.

Yes they replaced my teams expertise after we had won tens of millions from banks in just a few years without using lawyers with Halford Hall, a legend in her own lunchtime.
We had won the Turner's a circa £8m net settlement for a business worth about 50p - on a good day.
They thanked us by lying about us/me replacing us and shafting real victims like @BomberMorgan & then losing £100m plus claims hands down, securing £4m only from their new friends @LloydsBank & demanding they took it.
How...
They got their money including £1.1m blood money to get rid of me and then loaned that to victims, as long as they sacked me.
As I say Gould, Turner's, Buchanan, Hollinrake you all lied through your teeth, sue me...
I have the emails between Turner & White @LloydsBank COO, I have the evidence, I have Paul Turner emails helping gunnercooke lawyers to sack me.
Before the now suspended Nick Gould, who then went on to abuse vulnerable Post Office victims, took over the expectation of vulnerable bank fraud victims from my expert team.
He'd never dome a real deal in his life, had been making his living introducing @SMEAllianceLtd members to idiots at gunnercooke & then without disclosing it, taking a 10% plus fee split for the introductions.
Whilst stating repeatedly in @SMEAllianceLtd meetings he "hadn't earned a penny from members", outright lies...
We had 100 years of banking experience between us, I was the expert on the Rules, my friend a former Area Director at Barclays & our counsel an expert banking barrister, Monaco based & ex in house counsel at HSBC.
But why would you need our expertise when you had Georgina.

There is much more to it Artur.
How she was treated by @LloydsBank
How they tried to shut her up. I was working with Sally until the Turner's / @SMEAllianceLtd did a deal with @LloydsBank to get me off cases in the Griggs Review.
She was told along with others, don't trust Middleton, he's violent, incompetent, ripping victims off, we were the brilliant negotiators they said & Nick Gould from gunnercooke (now suspended over abuse of vulnerable PO victims) with the "Turner Method".
All lies. driven by greed from people who had barely worked a day in two decades. Despicable people who had taken over £30,000 from a dying parents bank account & then abused vulnerable HBOS Reading victims for their own financial gain.
A deal cut in a quiet room between Nikki Turner & Adrian White @LloydsBank COO, 'blood money' to get rid of the risk I posed and turn over the real victims.
Those are the facts, many now know that.
As Paul Moore said to corrupt @LloydsBank directors, you know who I am, you know where I am, sue me...
Gould sat reading a book all day at their mediation, his first. Before promoting himself as a banking & mediation expert the next day.
All lies backed by the same untrustworthy @kevinhollinrake you quote as supporting Sally. The politician who turned over 60,000 plus SME victims with the BBRS & then despicably used his failure to promote himself to @CommonsTreasury
If you had no reason to mistrust @toryparty previously you definitely have one now with its car salesman (sorry Estate Agent) Chairman @KemiBadenoch
He supported a completely incompetent Halford Hall - as Paul Moore said 'Sue Me' - to look after Sally's case instead.
I was told & suspect that didn't go well...

And funnily enough the banks paid out redress for the smaller claims in the hidden credit line scandal . The same hidden credit lines they refused to pay out to the large claims. Mis sold aka fraud
The Post Office Horizon Scandal and the Hidden Credit Line Scandal:
How Long Can Compelling Documentary Evidence Be Denied?
For years, Post Office executives and lawyers insisted that Horizon was reliable.
Even as evidence mounted that the system was defective, innocent sub-postmasters continued to be prosecuted, financially ruined, imprisoned and publicly discredited.
The institutional narrative was maintained. Those who challenged it were treated as the problem.
Now consider the Hidden Credit Line Scandal involving major Irish and UK banks — and the continuing position of the UK’s Financial Conduct Authority (FCA).
There is a deeply troubling parallel, but one crucial point must be emphasised.
The documentary evidence exists. It is extensive. It is compelling. And internal banking whistleblowers have corroborated the existence and scale of practices that destroyed otherwise viable businesses.
This is not simply a dispute about banking practices, mis-selling or unfortunate commercial losses.
The evidence raises fundamental questions about concealed credit arrangements, undisclosed collateral shortfalls, manipulated valuations, derivatives transactions, engineered defaults and the transfer of banking losses onto unsuspecting SMEs - so the foreseeable or deliberately engineered consequences of arrangements known to those operating them.
And the evidence does not come solely from the victims.Internal banking whistleblowers have corroborated the scale and nature of what was occurring. Documentary records provide a compelling evidential foundation for an independent forensic investigation.
Yet the @TheFCA continues to deny or minimise the existence and significance of the Hidden Credit Line Scandal.
What precisely is the FCA disputing?
The documents?
The internal whistleblowers?
The transactions?
Or the implications of accepting that viable businesses were systematically exposed to destruction?
The Post Office Scandal demonstrated how an institutional narrative could be maintained for years, even while evidence emerged that should have brought prosecutions and other enforcement action to a halt.
The parallels demand examination.
What did banking executives, lawyers, auditors and regulators know?
When did they know it?
What information was withheld from borrowers, courts and parliamentary investigations?
Why have documented concerns and corroborating whistleblower accounts not resulted in the comprehensive independent investigation they warrant?
And what responsibility arises where lawyers or other participants in litigation knowingly advance accounts contradicted by documentary evidence?
The United Kingdom has legal mechanisms capable of addressing fraud, false evidence, perverting the course of justice and other serious misconduct, where the statutory or common-law requirements are established.
These mechanisms are not rendered irrelevant simply because the alleged wrongdoing occurred within major financial institutions or involved powerful professional advisers.
The lesson of the Post Office Scandal is that institutional insistence is not proof, regulatory reassurance is not a substitute for evidence, and the destruction of innocent lives cannot be excused by repeating an official narrative.
The Hidden Credit Line Scandal requires an independent forensic investigation of the documents, the internal whistleblower evidence, the financial transactions and the role of those responsible for oversight.
Not another superficial review. Not another exercise in institutional self-exoneration.
The question for the FCA is increasingly difficult to avoid: if the documentary evidence and internal banking whistleblower accounts are as compelling as they appear, on what evidential basis does the regulator continue to resist acknowledging the full scale of the Hidden Credit Line Scandal?

‼️18 years ago this week‼️
🔹RBS was collapsing at the same time Fred Goodwin was reassuring investors - but what was happening behind the scenes at its Irish subsidiary, Ulster Bank?
🔹18 years on, some very serious questions remain unanswered.
🔹When did RBS Group Treasury first instruct Ulster Bank to reduce its Irish lending exposures?
🔹Were perfectly viable Irish SMEs, farmers, hoteliers & property owners identified for exit because RBS desperately needed to conserve liquidity and capital?
🔹Were collateral valuations manipulated to manufacture covenant breaches & trigger defaults?
🔹Were borrowers pushed into restructuring, insolvency or asset disposals to address the bank’s own financial difficulties?
🔹What happened to the interest-rate swaps that many contend were fraudulently sold to these customers?
🔹Were invalid break costs, undisclosed credit facilities & invalid derivative liabilities used to intensify financial pressure?
🔹Who authorised the transfer of loans and associated derivative claims, and were customers ever properly informed?
🔹Did RBS benefit from fees, distressed asset disposals, derivative close-outs or government-backed loss protection arising from these exposures?
🔹What did RBS Group Treasury, Ulster Bank’s board, its auditors and the regulators know & when?
🔹Shouldn’t the Irish Banking Inquiry have obtained & forensically examined the relevant treasury instructions, credit committee records, valuation files & derivatives documentation?
🔹How many innocent Irish businesses & families lost everything while the banking group responsible for their financial arrangements was being rescued by taxpayers?
🔹Have we ever properly investigated whether the financial distress of the bank was transferred onto unsuspecting customers & whether their alleged defaults were manufactured & accelerated by the bank itself?
🔹18 years later, the shareholders’ losses and taxpayers’ rescue costs are extensively documented.
🔹Where is the equivalent accounting for the businesses destroyed, homes lost, livelihoods extinguished & families devastated?
🔹 Again, did RBS’s access to taxpayer-backed loss protection under the UK Asset Protection Scheme create inappropriate or indeed dangerous financial incentives to classify Irish borrowers as impaired, restructure their loans or trigger defaults & were adequate safeguards in place to prevent banks from benefiting from customer distress they had themselves caused or aggravated?
🔹 If credible evidence existed that the RBS banking group’s financial pressures were being transferred onto unsuspecting Irish businesses & families through questionable valuations, manufactured defaults, invalid derivative liabilities or aggressive enforcement, why was decisive intervention not forthcoming?
🔹The evidence points to an abject failure to act on what was known and/or a conscious decision to acquiesce by Irish civil servants, politicians & regulators.
🔹 A transfer of banking losses onto private citizens on such a scale should have required explicit legal authority, transparent accounting, democratic consent & rigorous parliamentary scrutiny.
🔹Instead, were thousands of individual borrowers left to fight complex, expensive & often ruinous legal battles without ever being told that their financial destruction might actually have formed part of a much wider banking restructuring?
🔹Which law firms had “authority” to enforce & what role did certain judges play by bellowing out: “You borrowed the money!” while simultaneously ignoring documentary evidence?
🔹What exactly did Irish Ministers, senior civil servants & regulators know? When did they know it? And why was the possibility of such a transfer of losses not openly investigated?
🔹If this was, in substance, a covert bail-in, who authorised it, who facilitated it, who benefited & why was the Oireachtas never asked to scrutinise it as such?

SIX BANKERS JAILED
Sally Masterton was a forensic accountant at Lloyds Banking Group (@LBGplc). In 2013 she wrote an internal report, Project Lord Turnbull, documenting how HBOS executives had deliberately concealed a £1 billion fraud at their Reading branch before Lloyds acquired them.
Small businesses destroyed. Lives ruined. Fraud siphoning money to pay for kickbacks, prostitutes and Barbados holidays.
She was pushed out in 2014. Lloyds publicly denied the report was even authorised. They called her a rogue employee acting alone.
The police, meanwhile, were writing letters saying her work was vital to their investigation and trying to have her co-opted onto the case.
@TheFCA received the report in 2014. They did nothing for years. Nothing. The regulator whose entire job is to catch this exact thing sat on it.
Six HBOS executives were eventually jailed for a combined 47 years. The fraud was real. The cover-up was real. Masterton was right about all of it.
Lloyds settled with her twice. The second time, in 2018, they formally admitted the report was commissioned by the bank and apologised, saying she had acted with integrity and good faith. That is what institutions call justice when they want to avoid the word sorry.
The FCA was fined £45 million for concealing the fraud. Not a single board-level executive was held accountable. The £45 million fine went straight to the Treasury.
Thames Valley Police spent £7 million investigating a fraud they could never recover from the fraudsters. The Chancellor at the time refused to reimburse them.
A woman who did her job was fired, smeared, and silenced with a non-disclosure agreement. The people who covered up a billion-pound fraud kept their careers.
Nothing about any of this was an accident.
MP Kevin Hollinrake (@kevinhollinrake) called her treatment disgraceful. He was right. It was also completely predictable for anyone who knows how these institutions protect themselves.
Share this if you think the public deserves to know.

GRG & BSU - a small part of the story Non Core between @natwest & @LloydsBank - over 5 MILLION customers targeted for de-banking & asset stripping.
The Asset Protection Scheme = public money used to insure frauds, £600billion of customer lending to be off the books in 3 years by hook or by crook.
Mis-sold fixed rates before the crisis became Weapons of Mass Destruction, as SME's were forced to buy them & fix at 7-8% (with lending margin).

All happening at @hmtreasury controlled banks before the @bankofengland & @TheFCA coordinated to crash rates & destroy SME balance sheets.
Other banks followed suit, if the Government can get away with it, why can't we?
Then package the SME debts in bonds, alongside high credit quality loans, sell them to pension funds desperate for yields with rates and Gilts below 1%.
No more than 10-20% per bond/portfolio so no high default furore, as premium buyers flocked in for the Government backed 'gilt-edged' high yield products.
Manage controlled defaults at first & then accelerate to pass on the capital losses to the pensions & other investors.
Intentional cull of a generation of SMEs, hundreds of thousands of lives destroyed, hundreds if not thousands of suicides.
All because the regulators & Whitehall Mandarins did not want you to know the black holes were not £40-50 billion at the corrupt @cityoflondon they are so desperate to protect = It was more like £4-500 billion.
State sponsored theft to cover up the real extent of the bankers greed & incompetence.

Vulture fund Cerberus becomes preferred buyer, having no qualms over destroying fraud victims.
Why didn't the banks speak to us, why didn't they offer us the loans at 50% haircut?
Because the frauds were too many & too big, they had to destroy our generation of entrepreneurs, to destroy their claims & voices & do it quickly before we worked it out.
