Progress!
For the first time, a major US medical group says benzo tapers should start at just 5 to 10%. The American Society of Addiction Medicine's new guideline also says: → Physical dependence is an expected result of long-term use. It's not addiction. → Don't stop abruptly. → Tapering can take months to years. → Review use every 3 months. D Foster and John, both harmed by benzos, sat on the patient panel. They fought for two years to get these changes in. Every week someone asks me if they can sue the doctor who pulled them off too fast. Now patients have something solid to show their doctor. Comment TAPER and I'll send you my free safe tapering guide.

High LDL but Low Blood Pressure 🩸 One reason I might have no detectable soft or calcified coronary plaque (0 mm3), despite my historically astronomical LDL cholesterol (PMID: 42187880), is my low blood pressure. Here's a bit of biology: Atherosclerosis isn't simply about LDL circulating in the bloodstream then diffusing into the artery wall randomly. In other words, the endothelium isn't just a passive barrier. And it's exquisitely sensitive to mechanical forces. In straight arteries, smooth, laminar blood flow generates shear stress that promotes protective pathways, including nitric oxide production and anti-inflammatory signaling. However, when flow becomes disturbed and/or blood pressure increases, the variation in mechanical forces alter chemical signaling, and very likely transcytosis we all. So, it’s a reasonable hypothesis to state my low blood pressure is one reason my heart is as clean as a whistle. 🚨 The point: Atherosclerosis isn't just about the LDL particles. It's also about the metabolic environment. And, here are 6 other factors I care about when evaluating the metabolic environment in which my heart lives. What do you score? And have you download the free Beginner’s Guide to Metabolic Health yet? (Link 🔗 in quoted post👇) 💯% FREE. Not even an email gate. Share widely! P.S. I'll be delving into the topics in far more detail in upcoming deep-dive content.

As a mom of young adults this is terrifying. KMT can make addiction management & drug tapering so much easier.
Presley Gerber had every resource in the world. Loving famous parents, access to the best doctors money can buy, lots of people genuinely trying to help him. And a year before he died, he told us all exactly what was wrong, on Instagram: He'd been through 15 different psychiatrists. All any of them offered was different drugs. Nothing was working. And he was searching for one thing he couldn't find: someone to taper him off. Shortly after that, another reel: two different benzodiazepines, buprenorphine, and more. A cocktail. And a year later he's dead at a rehab, with the same combination that keeps appearing in these stories: benzodiazepines plus opiates. I sat down with Dr. Drew to talk about it, because the pattern has become impossible to ignore. Hayden Panettiere's toxicology is now out, per reporting: benzodiazepines, antipsychotics, fentanyl. Matthew Perry. Heath Ledger. Different decades, different famous faces, same cocktail. All the access in the world, and what it bought them was more prescribers, more prescriptions, and no exit. That's the detail that should haunt everyone: the thing Presley couldn't find in all of Hollywood wasn't another medication. It was a doctor willing to help him come off. Deprescribing is the rarest specialty in American medicine, and people are dying in the gap. Hollywood organizes around so many causes. The cocktail of sedatives and opiates is killing their friends and their children year after year, and it should be their number one issue. The silence tells you how completely the "this is just modern medicine" story has won. Full conversation at the link in bio. If you or someone you love is struggling, SAMHSA's free helpline is 1-800-662-4357. If someone you know takes both a benzodiazepine and an opioid, talk to their doctor now, and ask about naloxone. If you're looking to safely taper off a psychiatric medication, comment TAPER and I'll send you my free tapering guide. It's the exact protocol we use with our patients at TaperClinic. Never stop these medications abruptly. Any taper should be gradual and medically supervised.

We just had a resident move back into on of our assisted living homes after he moved back home about a year ago. Back then we improved his A1C from about 12 to 5.8, lowered his blood pressure and had him walking well. 🧵(1/8)

Here's a fact that should reorganize how you think about depression: after decades of brain scans, autopsies examining serotonin receptors, even studies putting needles in people's spines to measure serotonin metabolites, researchers have found no consistent biological marker for depression or anxiety. None. That's why there is still no lab test and no brain scan in psychiatry. Not because we haven't looked. Because it isn't there. Now compare that to what the research does show, consistently and powerfully: childhood trauma. Social isolation. Poor physical health. Lack of meaning and purpose. The correlation between problems in these areas and mental health struggles is strong and undeniable. Your grandmother could have told you this list. The data backs her completely. So look at America through that lens. Seventy percent of us are overweight, thirty percent obese. We're more socially isolated than any generation in history. We soothe ourselves with inflammatory junk food and screens. And our response to the mental health crisis this produces is to call it a biological disease and prescribe accordingly, while the actual drivers, the ones with the strong evidence, go unaddressed as a matter of national policy. Why do we keep looking in the wrong place? Because an entire industry depends on the answer being "it's medical, it's your brain, get on the drugs." There's no product to sell for loneliness. No pill margins in meaning and purpose. Imagine what we could do as a country if we put even a fraction of our mental health spending into the things that actually correlate with mental health. That conversation is sitting right there, and almost nobody in power is having it. If you're looking to safely taper off a psychiatric medication, comment TAPER and I'll send you my free tapering guide. It's the exact protocol we use with our patients at TaperClinic.

A new RCT shows that even when UPF meals are nutritionally matched to non-UPF meals, they trigger higher insulin spikes, alter energy metabolism, and also change how your brain responds to food cues. Food processing matters far beyond macronutrients! nature.com/articles/s42255-0…

This is one of the more concerning consumer-exposure studies I’ve seen in a while. 98% of personal care and household products contained at least one chemical that was not disclosed on the label, including several endocrine-disrupting chemicals such as bisphenols, phthalates, and parabens. 85% contained at least one chemical with a known or suspected health hazard, more than half failed testing because of undisclosed contaminants, and 26% contained chemicals that directly contradicted claims on the label. On average, the products contained more than 3x as many chemicals as their labels disclosed. And this included products marketed as “natural,” “non-toxic,” and “fragrance-free.” I’ve talked for years about reducing unnecessary exposure to endocrine disruptors. But you can’t avoid a chemical if you don’t know it’s there! Ingredient labels alone may simply not be enough at this point. We need much better testing and transparency of the products people are putting on their bodies and using in their homes every single day.

KMT may change medication exposure, and the direction may be drug-specific. journals.plos.org/plosone/ar… KMT is not just about adherence to the diet. The prescriber's behavior has to change too. Someone has to anticipate medication–metabolic interactions, interpret new symptoms correctly, obtain levels when appropriate, and adjust cautiously rather than assuming that the old dose means the same exposure in the new metabolic state. – at Vancouver, WA

Appreciate the summary!
A Major Trial Put Keto Against the Mediterranean Diet. Keto Won. 1/4) In a head-to-head showdown between a keto diet, a Mediterranean diet, and a low-fat plant-based diet, keto came out on top for several cardiometabolic outcomes. The researchers concluded: “These results demonstrate that a very low-carbohydrate ketogenic diet has greater cardiometabolic benefits [than matched Mediterranean and plant-based low-fat diets]” But there are some important nuances. Let’s get into them. (don’t miss 3/4) 🧵👇

If diabetes results from metabolic dysfunction, and heart disease results from metabolic dysfunction, why don’t we think more about mental illness through the same lens? The mind is extraordinarily complex, but it emerges from the brain, which is a purely biological and metabolic organ.
