High LDL but Low Blood Pressure 🩸
One reason I might have no detectable soft or calcified coronary plaque (0 mm3), despite my historically astronomical LDL cholesterol (PMID: 42187880), is my low blood pressure.
Here's a bit of biology: Atherosclerosis isn't simply about LDL circulating in the bloodstream then diffusing into the artery wall randomly.
In other words, the endothelium isn't just a passive barrier. And it's exquisitely sensitive to mechanical forces.
In straight arteries, smooth, laminar blood flow generates shear stress that promotes protective pathways, including nitric oxide production and anti-inflammatory signaling.
However, when flow becomes disturbed and/or blood pressure increases, the variation in mechanical forces alter chemical signaling, and very likely transcytosis we all.
So, it’s a reasonable hypothesis to state my low blood pressure is one reason my heart is as clean as a whistle.
🚨 The point: Atherosclerosis isn't just about the LDL particles. It's also about the metabolic environment.
And, here are 6 other factors I care about when evaluating the metabolic environment in which my heart lives.

We just had a resident move back into on of our assisted living homes after he moved back home about a year ago.
Back then we improved his A1C from about 12 to 5.8, lowered his blood pressure and had him walking well. 🧵(1/8)

Here's a fact that should reorganize how you think about depression: after decades of brain scans, autopsies examining serotonin receptors, even studies putting needles in people's spines to measure serotonin metabolites, researchers have found no consistent biological marker for depression or anxiety. None. That's why there is still no lab test and no brain scan in psychiatry. Not because we haven't looked. Because it isn't there.
Now compare that to what the research does show, consistently and powerfully: childhood trauma. Social isolation. Poor physical health. Lack of meaning and purpose. The correlation between problems in these areas and mental health struggles is strong and undeniable. Your grandmother could have told you this list. The data backs her completely.
So look at America through that lens. Seventy percent of us are overweight, thirty percent obese. We're more socially isolated than any generation in history. We soothe ourselves with inflammatory junk food and screens. And our response to the mental health crisis this produces is to call it a biological disease and prescribe accordingly, while the actual drivers, the ones with the strong evidence, go unaddressed as a matter of national policy.
Why do we keep looking in the wrong place? Because an entire industry depends on the answer being "it's medical, it's your brain, get on the drugs." There's no product to sell for loneliness. No pill margins in meaning and purpose.
Imagine what we could do as a country if we put even a fraction of our mental health spending into the things that actually correlate with mental health. That conversation is sitting right there, and almost nobody in power is having it.
If you're looking to safely taper off a psychiatric medication, comment TAPER and I'll send you my free tapering guide. It's the exact protocol we use with our patients at TaperClinic.

A new RCT shows that even when UPF meals are nutritionally matched to non-UPF meals, they trigger higher insulin spikes, alter energy metabolism, and also change how your brain responds to food cues. Food processing matters far beyond macronutrients! nature.com/articles/s42255-0…

This is one of the more concerning consumer-exposure studies I’ve seen in a while.
98% of personal care and household products contained at least one chemical that was not disclosed on the label, including several endocrine-disrupting chemicals such as bisphenols, phthalates, and parabens.
85% contained at least one chemical with a known or suspected health hazard, more than half failed testing because of undisclosed contaminants, and 26% contained chemicals that directly contradicted claims on the label. On average, the products contained more than 3x as many chemicals as their labels disclosed.
And this included products marketed as “natural,” “non-toxic,” and “fragrance-free.”
I’ve talked for years about reducing unnecessary exposure to endocrine disruptors. But you can’t avoid a chemical if you don’t know it’s there!
Ingredient labels alone may simply not be enough at this point. We need much better testing and transparency of the products people are putting on their bodies and using in their homes every single day.

KMT may change medication exposure, and the direction may be drug-specific.
journals.plos.org/plosone/ar…
KMT is not just about adherence to the diet. The prescriber's behavior has to change too. Someone has to anticipate medication–metabolic interactions, interpret new symptoms correctly, obtain levels when appropriate, and adjust cautiously rather than assuming that the old dose means the same exposure in the new metabolic state. – at Vancouver, WA

If diabetes results from metabolic dysfunction, and heart disease results from metabolic dysfunction, why don’t we think more about mental illness through the same lens?
The mind is extraordinarily complex, but it emerges from the brain, which is a purely biological and metabolic organ.

People ask me why I'm so passionate about cattle, and I walk away every time thinking I let her down.
Take the trick nobody else on this planet can do. A cow walks onto a hillside that will never grow a crop, a slope too steep, too wet, too thin, too rocky, the sort of ground that breaks a plough and bankrupts anybody who tries it, and she eats the only thing living up there. Grass. A substance from which no human being has ever extracted a single calorie. Then she stands out in the rain and turns it into steak.
She is doing the impossible half of agriculture, on land nobody else wants, in weather nobody else will work in, for nothing.
The machinery is properly deranged. Four chambers, the first a fermentation vat holding tens of gallons at a steady temperature, running a microbial population she farms inside her own body and feeds by hand. She sends the food back up and chews it again for eight hours a day, producing well over a hundred litres of saliva in the process, which is more fluid than most people drink in three months, entirely to keep the bugs comfortable. She is a brewery with legs, and she is also the brewer.
Those bugs make B12 out of cobalt and grass. She grows her own vitamins in her own stomach. No plant makes it, no human can make it, and every vegan alive is buying in a tablet what she produces for free, standing up, in a wet field, while thinking about nothing.
Then she gives the lot away.
Milk, a complete food that arrives warm. Butter and cheese, which is milk built to survive a winter. Beef, carrying the heme iron and the zinc and the retinol and the creatine in exactly the proportions you need, because the assembly was done by an animal built along the same lines as you. Tallow for the pan. Leather for your boots. And dung, which grows next year's grass, so the fertiliser plant sits on the same site as the meat operation and nobody drives anything anywhere.
Sunlight and rain in one end. The finest food available to a human being out of the other. On ground that grows nothing else. Using an engine that laboratories have spent a fortune failing to copy, in a warehouse, on a grant, with a press release.
For this she has been declared an environmental problem by people who have eaten her their entire lives and could not tell you what a rumen is.
I have never met a cow that was underperforming. I have met a great many committees.
Get her on your plate and be grateful.

I'm delighted to share the news of my upcoming book The Carb Crisis. Pre-orders have opened and you can find a list of vendors here: simonandschuster.com/books/T…. Thank you for any early interest and early orders.
The motivation for this book came from wanting to tell the other side of the story when it comes to dietary carbohydrates. Whereas Why We Get Sick focuses on insulin resistance (which is influenced by dietary carbohydrates), The Carb Crisis focuses more on the substance of the dietary carbohydrates directly, outlining the problematic molecules that come with these foods, as well as their effects in the body.
If you liked Why We Get Sick, you'll love The Carb Crisis.
