1/ Now the embargo has lifted, I’ve had time to read the @lengreview in full. While some celebrate minor cosmetic wins, I see the formal entrenchment of a parallel, underqualified medical workforce — built without medical training.

I heard this podcast on the weekend and feel it should be mandatory listening for all regulators and regulated professionals!
It's about a very senior civil servant who lived a blameless life until his late 30s, when he accidentally left top secret files on a train. Ultimately, he was demoted, lost his security clearance and got a conviction for breaching the Official Secrets Act.
There are many interesting nuggets in the podcast, but I was particularly struck by a few which could have come from almost any #FitnessToPractise case:
~ How he initially handled his mistake (with a lack of immediate candour) only made things worse and was a reminder that how you handle any mistake is often much more important than the error itself.
~ The feelings of absolute worthlessness and despair, and his serious contemplation of suicide (over an inadvertent error), is something which we see time and again. Regulators need to be reminded that the purpose of fitness to practise proceedings is not to punish (it's public protection). Often the regulated individual is the most vulnerable individual in the picture and needs to be treated accordingly - or, at least, treated humanely.
~ You can get through pretty much anything.

Prediction time- and a bookmark for those who fancy it.
There will be attempts to fudge the PA protocols and dilute by the pro “PA are just differently trained doctors” lobby.
Absolute lock on that Leng will be under heavy pressure to be part of that
Organisations such as Royal Colleges will try to manage it internally and “find a middle ground”
(Colleges will mostly do as asked- unless the uproar is too much to handle)
They will eventually come in the public domain
There will be uproar and much debate as Fellows and Members see them.
The NHS Long Term Workforce Plan will try- again - to shoehorn PAs in.
The basic strategy?
“Let’s find a middle ground”- based on a belief that people are tired of this and will accept.
And a hope that enough has been given to the BMA - pay structures, IMG sacrifice etc for them to sign up
As ever?
The key sits with the supervisors.
Will YOU as a Doctor- agree to taking lesser trained people on and put your liability on the line.
And will you have the time once METR lands.
Policy is one thing - involves much of politics
Implementation?
Needs wide support.
Ask those who tried to force the whole PA project through first time around.

Dozens of bot and anti-trans campaigners from a single post suggesting we don't encourage harassment. Accused of misogyny when I've spent a long time supporting women in medicine.
Obviously I have to lock my account for a bit to deal with these vile individuals.
It is interesting that I retweet many things that do not affect my algorithm, yet when I QT one very contentious topic, my feed becomes inundated with that issue.
It's almost as if a certain billionaire is using this platform to amplify the issues driving discontent in society.
Leave aside views on women's and trans rights for a moment.
This is just gross harassment. It is a matter of time until such action leads to another suicide.
Comments going off, obviously, because we all know what happens with this issue and the right-wing bots..

I was always intrigued by Prof Leng not going through with having a defined SoP. Instead this has shifted to trying to define 'clinical protocols' with 84 stakeholders.
Leave it to expert organisations to define. Enforce it via the GMC. Then have timely reviews 🤷♂️
Response to @RCPhysicians ask re @lengreview clinical protocol done- as a Fellow of RCP.
I believe in transparency- so here are my views on some of the key questions.
In summary?
Needs a rejection on key areas of PAs seeing undifferentiated patients.
And certainly not lean on Gen Next to be liability sponge.
And let @rcgp make their own decisions regards primary care- no need to interfere.
NB:
Don't like it, give a counter view.
Don't abuse or be snarky. It's a democracy- it's my view. You are welcome to give yours.

A stunningly well-composed and evidenced thread. I would expect nothing less from someone who has suffered firsthand from maternity failings. Perhaps we ought to hear him out?
There is an appalling narrative at the moment (driven by a small but very powerful group) who claim that ‘normal birth’ ideology & its impact on safety is a made up issue.
A reminder of the vast amount evidence there is on this.

100% false. The base did not “burn down.”
FACT: Thanks to a coordinated effort, just 1 structure was lost.
FACT: The firefighting effort on the base is led by federal first responders — supported by local and state resources — given it’s a military base.
Spreading misinformation about emergency situations is disgusting and unhelpful.
Thanks @CAgovernor for letting a military base burn down. This is one of the Marine Corps most important installations and you couldn’t keep it safe.
Readers added context they thought people might want to know
Camp Pendleton did not burn down. The Henderson Fire burned more than 3,132 acres on the base. Times of San Diego reported no structural damage, and the base's own Camp Pendleton Fire Department fought the fire with Cal Fire. nbcsandiego.com/news/local/i-5… timesofsandiego.com/crime/2026/10/…
