Patients with estrogen-receptor-positive (ER+) HER-2-negative advanced breast cancer experienced significantly improved progression-free survival when treated with an oral combination regimen that included giredestrant, a novel, next-generation selective ER degrader and full antagonist, compared to a standard combination approach.

Approximately 10% of invasive breast cancers are classified as invasive lobular carcinoma (ILC), but this distinct subtype has received limited attention in clinical trials and lacks an established treatment strategy. 🩷✨ A new phase III signal in endocrine-resistant breast cancer, and one of the strongest effects appears in ESR1-mutated disease.
The evERA Breast Cancer study has now reported detailed phase III results for giredestrant + everolimus after progression on prior CDK4/6 inhibitor + endocrine therapy in ER-positive/HER2-negative advanced breast cancer.
📊 In the overall population:
➡️ Median PFS: 8.8 vs 5.5 months
➡️ HR 0.56
➡️ 44% lower risk of progression or death
🧬 In ESR1-mutated disease:
➡️ Median PFS: 10.0 vs 5.5 months
➡️ HR 0.38
➡️ 62% lower risk of progression or death
💊 The combination pairs a next-generation oral SERD with mTOR inhibition, targeting two major mechanisms involved in endocrine resistance.
📈 Overall survival is still immature, although the current analysis shows a favorable numerical trend:
• ITT: HR 0.69
• ESR1-mutated: HR 0.62
🔬 The broader message may be just as important as the PFS result:
ESR1 is increasingly becoming an actionable treatment-selection biomarker, not simply a marker of endocrine resistance.
As the post-CDK4/6 landscape becomes more crowded, the next treatment decision may increasingly depend on why endocrine therapy stopped working.
⚠️ Giredestrant remains investigational in this setting, with an FDA decision on the advanced-disease application expected in December 2026.
