El sistema de salud mexicano se sostiene por estudiantes: médicos residentes a los que se les paga una miseria y llegan a trabajar hasta 72 horas a la semana. En México nadie es más explotado que un residente o un interno.

🧬 Can we more than double progression-free survival in oligorecurrent hormone-sensitive prostate cancer by combining targeted radioligand therapy with SBRT? ascopubs.org/doi/10.1200/JCO… 177Lu-Prostate-Specific Membrane Antigen Neoadjuvant to Stereotactic Ablative Radiotherapy ☢️ for Oligorecurrent Prostate Cancer (LUNAR) The phase II LUNAR 🌜trial investigated whether adding PSMA-targeted radioligand therapy (¹⁷⁷Lu-PNT2002) before stereotactic body radiotherapy (SBRT) improves outcomes in oligorecurrent hormone-sensitive prostate cancer. Among 92 randomized patients, combination therapy significantly prolonged median progression-free survival from 7.4 to 17.6 months. @AmarUKishan, @CalaisJeremie, @theboutroslab, @wayne_brisbane, @JoanneWeidhaas, @kristenmcgreevy, @SophiaParmisano @OncoAlert 🚨 #LPCCC27 @Silke_Gillessen @AOmlin @ProfKHerrmann @weoncologists

Ok. I’ve spent a whole day celebrating Karl Deisseroth’s very deserved Nobel (with every other brain enthusiast!). Now let’s talk about the brilliance of Dr. Michelle Monje, his wife. A Stanford physician, neuroscientist and recipient of a MacArthur Genius grant, Michelle has helped pioneer cancer neuroscience, revealing how brain activity can fuel tumor growth and opening new possibilities for treatment. And I say all this because I’ve been lucky to see her work firsthand and have been awe struck since the moment I met her. What makes Michelle extraordinary is the combination of scientific brilliance and her absolute unwavering commitment to children with devastating brain cancers. She is pushing the science forward because those children and their families cannot afford to wait. Everybody deserves to know @michelle_monje.

Brain cancers take advantage of neural circuit activity. Re-purposing an anti-seizure med called levetiracetam disrupts an important neuron-cancer interaction in #DMG #DIPG, slowing growth and extending survival in mice and retrospective patient data. nature.com/articles/s41591-0…

What if exercise wasn’t just “good advice” for men receiving ADT—but a core part of prostate cancer treatment? 👇 thelancet.com/journals/lanon… An integrated lifestyle intervention for mitigation of adverse effects associated with androgen deprivation therapy for prostate cancer (STAMINA): a multicentre, randomised trial The STAMINA randomised trial evaluated whether a 12-month lifestyle intervention could improve quality of life and fatigue in men with #ProstateCancer receiving androgen deprivation💊 therapy. ➡️Across 15 NHS🇬🇧 trusts and 700 participants, supervised aerobic and resistance exercise, dietary advice and behavioural support significantly improved prostate cancer-specific quality of life and reduced fatigue compared with Optimised Usual Care. ➡️The intervention was feasible within NHS cancer services, with no treatment-related deaths reported. @LiamBourkePhD, @SophieReale, @IbeSaid, @Alisonscope, @gracecrowther1, @DrDylanM, @DMeadster, @MCollinson_, @amanda_horler @OncoAlert @Silke_Gillessen @AOmlin @ProfKHerrmann @weoncologists

How can you NOT believe in God when octopuses dream and otters have a favorite rock they carry around in a little skin pocket and cows have best friends and bees are out there communicating the coordinates of flowers through dancing and they have regional dialects? Cats are purring at frequencies associated with bone healing? Forests get absolutely incinerated and respond by growing flowers? Dolphins literally invent names for themselves and yell them across the ocean? Penguins are waddling around Antarctica looking for the perfect rock to give their partner? CROWS REMEMBER YOUR FACE. THEY TELL OTHER CROWS ABOUT YOU. THEY TELL THEIR CHILDREN ABOUT YOU. YOU CAN HAVE GENERATIONAL BEEF WITH A BIRD. I NEED YOU TO UNDERSTAND THE SIGNIFICANCE OF THIS. Goats have accents. Rats laugh when you tickle them. Elephants mourn their dead. Whales make songs and then OTHER WHALES LEARN THE SONG and suddenly everybody across the ocean is singing the new song. Monarch butterflies are born knowing how to fly thousands of miles to a place they have never been. Sea turtles hatch out of an egg, immediately sprint toward an ocean that they have never seen, disappear into it for like 3 decades and then somehow come back to the same beach they hatched on. Birds can see colors that we humans can’t see. There are colors out there right now that we don’t even get to know about. There are sea slugs that eat algae and use the chloroplast/chlorophyll and then are just like…photosynthesizing. And they’re super adorable. There are mushrooms communicating underground. There are seeds sitting in the dirt for decades like “no actually I will wait. It’s fine.” Squirrels plant trees because they forget where they put their snacks. Relatable. A CATERPILLAR GOES INTO A LITTLE BAG THAT IT CREATES ITSELF AND DISSOLVES MOST OF ITS BODY INTO ORGANIC GOO AND REARRANGES ITSELF, GROWS WINGS AND THEN LEAVES. WHY IS THIS NOT MORE BEAUTIFULLY TERRIFYING TO PEOPLE?? And you’re telling me a tiny acorn knows how to become a huge tree? It just has a blueprint inside itself? Your body started as ONE CELL. ONE. And that cell divided itself into a brain that can think about its own existence and eyes that can look at stars and hands that can hold somebody else’s hands and a heart that runs on electricity without you even asking it to? The iron in our blood was made inside a star?? We are walking around on a wet rock covered in bioluminescent oceans and singing whales and laughing rats and birds holding lifelong grudges and flowers exploding out of fire and somehow we’re supposed to act like this is a normal place to live? We’re supposed to act like this is all just a random, accidental sequence of events?

New in Advances in Radiation Oncology: Proximal vs distal isocenter targeting in SRS for trigeminal neuralgia This study asks a practical question: when you treat TN with radiosurgery, does it matter whether you put the shot at the dorsal root entry zone (proximal) or the retrogasserian zone (distal)? What they did: retrospective review of 86 patients treated 2017 to 2021, all prescribed 85 Gy to the isocenter, all with at least 1 year of follow-up. 54 proximal, 32 distal. Patient-reported pain relief was graded complete, partial, or none. Key findings: - Any pain relief: 90% distal vs 74% proximal - Complete pain relief: 46% proximal vs 31% distal (overall response distribution differed, p=0.011) - Complete medication discontinuation: 39% proximal vs 13% distal - Any medication de-escalation: 88% distal vs 72% proximal (p=0.001) - Median duration of relief: 37 months proximal vs 24 months distal, not significantly different (p=0.18) - No radiation necrosis and no patients treated for adverse radiation effects Why it matters: TN patients are not asking for a response rate, they are asking to stop carbamazepine and stop being afraid to brush their teeth. Complete relief off medication and durability are the endpoints that map to that, and this paper reports both. Important caveat: every proximal case was Gamma Knife and every distal case was frameless LINAC. Target location and delivery platform are completely confounded here, which the authors state plainly. Dose rate, falloff, and platform-specific brainstem constraints all differ, so this compares two strategies, not two anatomic targets. Toxicity was also not systematically captured, and facial hypesthesia is the complication that would most likely differentiate these targets. Bottom line: this is a useful real-world signal that proximal targeting may favor complete, medication-free relief while distal targeting may favor any improvement. It is hypothesis-generating, not practice-changing. The question worth answering prospectively is proximal vs distal on a single platform, with standardized pain scoring and systematic hypesthesia reporting. doi.org/10.1016/j.adro.2026.… @ASTRO_org #RadOnc #OncTwitter
