I don’t believe the “40% of med students don’t plan to practice” stat…but even if I did, here’s some more relevant stats.
- the government has cut RVU reimbursement year over year, while inflation has only increased…in the past few years alone we took a double digit pay cut because of government @MedicareGov reimbursement when adjusted for inflation
- private equity has taken over healthcare, fairly unchecked. This results in bad patient care, unstable healthcare markets and poor working conditions for physicians…
-healthcare administrators have grown 3800% versus physician #s growing 200% since the 70s. This represents declining physician autonomy and an overwhelming administrative burden…much of which is created by government programs like CMS “Quality Measures” which are less about care quality and more about gamesmanship and documentation burden
- insurance companies have a chokehold on the finances of healthcare…and their lobby dollars fund the congressmen who help keep them in power and allow them to continue to squeeze physicians and patients both
How ‘bout you work on some of those issues, Congressman…and get back to the med students after you do?
#1 reason for MD shortage is Med schools admitting students who aren’t going to practice. Only 60% of med students today plan on practicing clinical medicine. Med Schools need to focus on admitting students who want take care of patients, not just get their MD.
Well done Dr. Dr. Danny! Quite the research… dynamo!!
Check out our latest paper on functional outcomes for post-RP high-risk PCa! Most recover continence by 12 months but sexual function recovery is poor (15%). Thanks to the team @asemerjian @Kyle4Johnsons @KBGinsburg @cancerdrlane @MUSICUrology

Incredible year working with @SmigsM and the rest of the @CmcUrology crew! Learned a ton, met some phenomenal people, and had fun along the way. Would definitely recommend
Great last day of operating with our minimally (maximally) invasive fellow @Kyle4Johnsons
Look forward to seeing you around as an attending next year

And, There are fewer than 25k med students in the USA. All in with expenses it’s less than 100k a year for all but the most expensive schools.
So for less than 2.5b dollars a year we could make med school free. Limit any increases to less than COLA increases and it pays for itself quickly.
And guess what , make it free, you eliminate a cost barrier and you will increase the number of applicants so that in the future you will see an increase in the quality of applications beyond the rich or those who are willing to take on loans, improving diversity as well
Dumb. Really dumb. “The only way to create a successful career at the NIH “. So now you think working for a government agency is the path of “the renaissance man” in medicine?
And you think the Harvard path is the only way to success ? And what UCLA does means everyone does it ?
Dumbbbb
How about this.

And for your “Renaissance Men” there is so much VC money looking to use AI to change medicine and care, they don’t need the NIH. In fact , smart VCs and companies will start paying for their med school in exchange for a 5 year commitment. It’s cheaper than competing for them, recruiting and hiring them after their residency

Couldn’t ask for better co-chiefs ❤️ @CatherineSNamMD @MahirMaruf @alexzhuDO
Proud of our negotiations team and staff along with @umichmedicine’s negotiation team to reach a deal beneficial to both parties. Excited to watch the @hoaumich continue to positively represent residents and fellows at Michigan.
My watch has ended.
With a vote of 896 to 7 the UM House Officers Association ratified a 4-year deal with @umichmedicine
This agreement demonstrates both parties’ commitment to recruiting and supporting top-tier resident & fellow physicians who provide world-class care to the Michigan community.

A history of @theNRMP match: 🧵
The Match was established in 1952 when available resident positions vastly exceeded the number of graduating medical students. As a way to secure top students as residents, hospitals were:
1. Offering positions earlier and earlier, sometimes even prior to a student’s clinical years
2. “Exploding” offers and demanding a acceptance or rejection of an offer within minutes.
The labor market is different now. The match has also created an economic monopsony where there is one buyer (the match) and many sellers (student).
As a result, you see:
1. Wage suppression: The match eliminates any negotiation on salary or work conditions — with one “buyer”, they functionally control (and suppress) the wages of residents.
2. Lack of negotiation power: Once matched, students are obligated to accept the employment terms laid out by the program. This limits the ability of residents to negotiate better working conditions or benefits, which is a common practice in almost all other labor markets.
3. Limiting choice: Because candidates and programs must rank each other prior to the match, there is no opportunity for a student to decline an offer from a less-preferred program if they match there. This gets increasingly complex with scenarios like couples match.
4. Waste. Application fees, interview costs easily rack up thousands of dollars more of debt as students are incentivized to apply/interview at as many programs as possible to be able to rank more.
Economics aside, the match strips students of agency - to choose, to negotiate, to refuse…it treats students as widgets, interchangeable as just a body to fill a spot as determined by a faceless algorithm…
The “widgetization” of doctors is something we are well familiar with (ahem, CMGs…).
No wonder we end up how we do, burned out, treated as dispensable and replaceable by corporations, when this is how we start our careers.

Please help put pressure on the admin by writing your legislators.
Competitive salary is vital to recruit top-tier talent to UM. Michigan Medicine will struggle to provide great patient care to the community without the best resident physicians available

Sheesh... @nytimes would love to share some stories of ongoing negotiations between @hoaumich and @umichmedicine. This article is just the tip of the iceberg when it comes to resident salaries while delivering lifesaving care for 80 hours a week
In Opinion
"The rhetoric that a job is a passion or a 'labor of love' obfuscates the reality that a job is an economic contract," the author Simone Stolzoff writes in a guest essay. "The assumption that it isn’t sets up the conditions for exploitation." nyti.ms/3qtD8yZ

The @hoaumich sent a fair, reasonable contract across the table. Take a moment for some inspiration: youtu.be/9W9krBAOZs4?t=563

We hope this means some progress can be made at our next session. To date the HOA has been completely transparent. Admin continues to play the incremental shell game and seems to be running out the clock
A new cohort of residents starts in less than a month, Let’s get this done!

It is our pleasure to announce Dr. Kristian Stensland (@stensy) is joining our faculty this July. A former @LaheyUro resident, he joins after completing his @UroOnc fellowship at @UMichUrology. He is a surgeon-scientist whose research centers on implementation and clinical trials
