I was talking to a home health company and they were telling me how important it is to fall-proof an elderly person’s home with bars on the wall, removing slippery surfaces, and making sure there’s good lighting. “All those are good ideas” I said.” But what about getting them stronger and work on their balance?” They looked at me like I had two heads.
Them: Why do you lift weights at 72?  Me: Because gravity never retired.  Them: Isn't that risky at your age?  Me: Bones & muscles respond to heavy loads. Hiding from the weight is the riskier plan.  Them: How heavy?  Me: Heavy enough to be a conversation, not an argument.  Them: And when it's hard?  Me: I remember who's been carrying me. I can carry something for a few reps.

Interesting! Appreciate the summary.
A Major Trial Put Keto Against the Mediterranean Diet. Keto Won. 1/4) In a head-to-head showdown between a keto diet, a Mediterranean diet, and a low-fat plant-based diet, keto came out on top for several cardiometabolic outcomes. The researchers concluded: “These results demonstrate that a very low-carbohydrate ketogenic diet has greater cardiometabolic benefits [than matched Mediterranean and plant-based low-fat diets]” But there are some important nuances. Let’s get into them. (don’t miss 3/4) 🧵👇

Metabolic health benefits the most when insulin comes down the most. A new clinical trial tested three popular diets, and the diet that lowered insulin the most delivered the greatest metabolic payoff. Liver fat fell almost 50% more on the low-carb diet than on the others (low-fat plant-based and Mediterranean). The liver also became two to three times more insulin sensitive, produced less new fat from carbohydrate, and suppressed glucose production more effectively. Insulin across the day dropped far more with carbohydrate restriction, glucose fell more than twice as much, A1c improved the most, and prediabetes (i.e., insulin resistance) reversed at the highest rate. That is the beneficial biochemistry of a lower-insulin state. When insulin falls, the liver is no longer pushed to make and store fat. Lipogenesis slows, glucagon can tell the liver to oxidize fat instead, and the tissue becomes more responsive to the insulin that remains. The body becomes more insulin sensitive, and the liver can finally burn the fat it has been holding. Even though total weight loss was comparable across the three groups (by study design), lower insulin is what let the liver let go of the fat. cell.com/cell-metabolism/ful…

Help Christy find the person who saved her life.
Replying to @charlotte_dawn_
@charlotte_dawn_ Very weird question. But were you friends with the person in the comment section picture? I’d like to thank them for literally saving my life. As much as I rolled my eyes at their suggestion that food can heal….I tried it anyway! Free since then!

There isn't an easy answer. Many of the major neuropsychiatric disorders, including depression, schizophrenia, bipolar disorder, alcohol use disorder, Alzheimer's disease, and epilepsy, have been associated with increased risk for glucose hypometabolism in different brain regions. The challenges include the following: 1. Brain energy metabolism changes throughout the day, so sometimes it might be within normal limits and other times it might be reduced compared to controls. 2. Brain glucose hypermetabolism can also occur, likely representing hyperexcitability of neurons, which can also represent abnormal brain function. For example, this has been shown in manic episodes and specific circuits related to OCD. So... it's not as simple as reduced brain glucose metabolism. Instead, I would say it's dysregulation of brain energy metabolism, resulting in either underactivity or overactivity of neurons. Some scientists will argue that the metabolic changes are a consequence of neuronal activity, which may very well be true some of the time. The more disruptive concept is that the changes in metabolism, and more specifically mitochondrial function, may be driving neuronal activity.

The growing evidence that GLP-1 RAs can help with substance abuse (new article on the topic published today:pubmed.ncbi.nlm.nih.gov/4193…) shows the best use when it comes to weight loss. These drugs should be viewed as a tool to help people control consumption of the most addictive foods--refined carbs. Essentially, the drug can help a person better adhere to a low-carb diet, making weight loss easier. Furthermore, with this view comes a more deliberate focus on high-protein foods (along with the fat that naturally comes with them).

If Professor Bikman is cautious, then I listen.
This highlights one reason I’m so cautious with focusing too much on the microbiome. As much as we are learning, there are still many hurdles to overcome.

As Japan-USA relationships are reaching a new all-time high, I'm loving every minute of it. As a metabolic scientist, I've loved the opportunities to speak at events in Japan. Why would an American metabolic scientist be invited? Because the metabolic problem in Japan isn't that far off from the USA. To understand this, you need to appreciate the nuanced relationship between obesity and diabetes. We often assume that fat mass matters most. However, the size of our fat cells matters more when it comes to metabolic health. Caucasians (the dominant ethnicity in the USA) have the ability to make new fat cells as they gain weight. This means their fat mass can expand while the size of the fat cells remains fairly normal. However, in East Asians, the ability to generate new fat cells is limited; thus, any fat gain pushes the fewer fat cells to be larger. And large fat cells promote metabolic dysfunction. This explains why the USA can be significantly fatter than Japan, yet have diabetes rates that are only a point or so higher.

🚨Our Landmark Sport Nutrition Analysis in Endocrine Reviews — spanning 100+ years & ~600 studies🔬— challenges the foundations of the sports nutrition industry & guidelines, exposing potential harm to athletes' health. This is the most comprehensive pub on nutrition's impact on performance (5+ years in the making). What we found is critical to athletes, coaches, dieticians, and the scientists who write guidelines with FREE study link below: 1) 🍞"Carb-loading" is misguided? Current sport nutrition guidelines (i.e., @ACSMNews, @IntSocietySN, @Gatorade Sport Science Institute) recommend 5–12 g/kg CHO/day and 60–90 g CHO/hour. What does that mean? 📈This can push some athletes to over 1000 carbohydrates/day. Multiple meta-analyses in the report revealed that sport nutrition guidelines have over-emphasized muscle glycogen and carbohydrate oxidation levels...yet these metrics didn't consistently align with performance... so what metric predicts performance? See #2. 👇 2)🧠Primary performance driver? Sustaining total brain energy (glucose + ketones + lactate)—not muscle glycogen or carb oxidation. In >160 sports performance studies, 88% showed that carb intake only benefits performance when blood glucose plummeted in the placebo group, triggering early fatigue in non-carb athletes due to brain energy deficit. Carbs maintained blood glucose and brain energy, and this out-predicted every other metric. Yet, the brain's role has been sidelined in sports nutrition, ignoring evidence since the 1920s of its pivotal impact on performance. 3)🔥High carbohydrate intake paradoxically accelerates glycogen breakdown and suppresses fat oxidation during exercise. This contradicts many of the sports nutrition marketing claims promoting high levels of carbohydrate supplementation (60-120g/hour) to "spare" muscle glycogen AND evidence showing that higher fat oxidation correlates with better performance. 4)🫀Health risks of sports nutrition guidelines? Emergent evidence has demonstrated that high-carbohydrate intake can lead to prediabetes in a percentage of athletes. This analysis also reveals that current sport nutrition guidelines' carbohydrate intake levels create a metabolic environment during exercise analogous to diabetes: -⬆️circulating insulin -⬇️fat oxidation -⬆️glycogen breakdown -🔒forced reliance on glucose as fuel This is critical. We show that athletes ARE NOT immune to the metabolic problems driven by the food environment. 5)🥵Ketogenic athletes CAN "bonk" too...but not if they fuel correctly. Contrary to claims, athletes on ketogenic diets can and do bonk—like on high-carb diets—due to dropping blood glucose. The lack of strategic carbohydrate placement was a mistake many keto-athletes made when trying to reach their peak performance. Small targeted brain-fuel supplements (10g/hour) during exercise boost peak performance without high-carb loads. After 4-week adaptation, ketogenic performance equals high-carb performance...but BOTH high and low-carb athletes benefit from ~10g/hour carbs to maintain glucose/brain energy for exercise >60 minutes in duration, improving performance 22%. It will be interesting to see how other brain fuels can also assist (ketones and lactate). 🔗FREE Study Link: doi.org/10.1210/endrev/bnaf0… 📣CONCLUSIONS: This landmark analysis challenges over half a century of scientific assumptions in sports nutrition—and explains how they became embedded in official guidelines—with implications reaching beyond elite athletes to public health and chronic disease prevention. Please share this widely with athletes, coaches, dietitians, and scientists to help optimize not just peak performance, but lifelong health. 🤝

In just weeks, the new U.S. Dietary Guidelines for Americans (2025-2030) will be released, and what they include will shape the health of 340+ million Americans. This will be—by far—the most important reform to diet & health under this administration. A crucial part of any change must be a low-carbohydrate option for people with chronic diseases. The Guidelines drive food in school lunches, military mess halls, and ‘food baskets’ for women and infant children. The Guidelines are also considered the “gold standard” for doctors, nutritionists, and other professionals. Americans have followed the guidelines for the past 45 years (see infographic)—and clearly, our health has gotten dramatically worse.
