This is where complete evidence — p, fragility, robustness, plus absolute effect size — earns its keep. A p-value alone can’t tell a knife-edge interim result from a settled one. Here all four align: p<0.001; stable classification (SFQ 0.82, z = 6.5, nowhere near the significance boundary); far from no effect (SRQ 0.46, HR 0.42); and the absolute gain is clinically meaningful: +8.4 months median OS, 12-month survival 79.4% vs 53.3%. Statistical evidence is fully concordant, supporting the claim. Contrast this study with other unstable ‘significant’ trials that flip on one patient. Same p<0.05 culture, very different evidence. This study appears solid. This is great news.

🔬 Can adding an androgen receptor inhibitor to ADT and radiotherapy improve metastasis-free survival in very high-risk localised prostate cancer? New Phase 3 interim results from DASL-HiCaP suggest a significant benefit.
📊 LBA4 – DASL-HiCaP (ANZUP1801): Darolutamide with ADT and definitive or salvage locoregional radiotherapy
cslide.ctimeetingtech.com/es…
Darolutamide added to ADT and locoregional radiotherapy significantly improved metastasis-free survival in patients with very high-risk localised or biochemically persistent/recurrent prostate cancer. At four years, metastasis-free survival was 95% versus 88% with placebo (HR 0.42; p=0.0001).
✅ These findings support treatment intensification, although longer follow-up is needed to establish whether this benefit translates into improved overall survival. #ProstateCancer

🔬 Can perioperative immunotherapy plus an antibody–drug conjugate change the treatment landscape for cisplatin-ineligible muscle-invasive bladder cancer? New Phase 3 data suggest it could.
🧬 Durvalumab, Tremelimumab, and Enfortumab Vedotin in Muscle-Invasive Bladder Cancer
🔗 nejm.org/doi/full/10.1056/NE…
📊 The Phase 3 trial demonstrates significant improvements in event-free survival with perioperative durvalumab plus neoadjuvant enfortumab vedotin, with or without tremelimumab, compared with radical cystectomy alone in cisplatin-ineligible or cisplatin-declining patients with muscle-invasive bladder cancer. The durvalumab–enfortumab vedotin combination also significantly improved overall survival.
✅These findings highlight a promising perioperative treatment strategy, although the substantial incidence of grade 3 or higher adverse events warrants careful consideration.

A new post–PD-(L)1 option in advanced #RCC — but the story is more nuanced than PFS alone.
@DanielHengMD 🇨🇦 presents the final phase III analysis of belzutifan + lenvatinib vs cabozantinib:
📊 PFS: 14.8 vs 10.8 mo
HR 0.74
🎯 ORR: 53% vs 40%
⏳ DoR: 22.0 vs 11.9 mo
📈 OS: HR 0.85 — not statistically significant
🎯 Why this matters: belzutifan + lenvatinib delivers deeper and more durable disease control after prior immunotherapy, but without a demonstrated OS advantage.
For me, the clinically interesting question now is not simply whether this is another active post-IO regimen.
It is where it should sit in sequencing — relative to cabozantinib and other increasingly available options — and whether biology can help identify the patients most likely to benefit from HIF-2α + VEGF inhibition.
Excellent breakdown by @DanielHengMD.

The most interesting part of the 6-year NATALEE update is not only that the benefit persists — it is that the gap in invasive disease-free survival continues to widen after ribociclib has stopped.
Dr. Gil Morgan @OncoAlert breaks down the new data:
📊 OS: HR 0.81
➡️ 2.1% absolute benefit at 6 years
📊 iDFS: absolute benefit now 5.3%
✅ No new safety signals
⏳ Patients are now more than 3 years off ribociclib
🎯 Why this matters: adjuvant therapy asks patients to accept treatment and toxicity today for a benefit that may only become fully visible years later.
For me, the key question is whether the widening reduction in recurrence will ultimately translate into a durable survival advantage with longer follow-up.
The OS analysis remains descriptive and the absolute difference is still modest — but this is exactly why mature follow-up matters.

Dr. Gil Morgan (@OncoAlert) breaks down the 6-year NATALEE update!
In this episode of #OAOnTheGO, Dr. Gil Morgan, OncoAlert representative, breaks down the 6-year NATALEE update, simultaneously published in Annals of Oncology.
🔬 Key takeaways:
📊 Overall survival: HR 0.81, with a 2.1% absolute benefit at 6 years.
📊 Invasive disease-free survival: Absolute benefit has widened to 5.3%.
📊 No new safety signals, with patients now more than 3 years off ribociclib.
These findings strengthen the role of adjuvant ribociclib for eligible patients with high-risk, HR-positive, HER2-negative early breast cancer.
While the overall survival analysis remains descriptive and the absolute benefit is modest, the results bring an important question into focus: Can preventing more recurrences ultimately help more patients live longer?

Can a PD-1 × VEGF bispecific change first-line treatment for biliary tract cancer? 🤔
OncoAlert´s own Dr Vilma P. Barcia🇪🇸 (@DrVilmaPBarcia), Head of Medical Oncology at @Hospital_LaLuz (@quironsalud, GI Cancer Unit, #OneOncologyMadrid), to break down HARMONi-GI1, presented at the ESMO 2026 Presidential Symposium.
🔬 The study
Phase 3 trial of ivonescimab, a bispecific antibody targeting PD-1 and VEGF, plus chemotherapy versus durvalumab plus chemotherapy as first-line treatment for advanced biliary tract cancer.
📊 Key results
✅ Median OS: 14.9 vs 12.1 months (HR 0.75)
✅ Median PFS: 8.4 vs 5.8 months
✅ Higher response rates
⚠️ Points to consider
🔸 More grade ≥3 adverse events with ivonescimab
🔸 The trial population was exclusively Chinese, so validation in other populations is needed

Interim analysis; median event-free survival (EFS) follow-up: 22.9 months.
➡️ D + EV: EFS HR 0.51 (95% CI, 0.38–0.67); OS HR 0.58 (95% CI, 0.40–0.83)
➡️ D + T + EV: EFS HR 0.61 (95% CI, 0.47–0.80); OS did not meet statistical significance
➡️ Pathological complete response (pCR): 45% with D + EV vs 10% with control
➡️ Grade 3/4 treatment-related adverse events (TRAEs): 30% with D + EV vs 42% with control
✅ Both primary EFS endpoints were met. Perioperative durvalumab plus 3 cycles of neoadjuvant EV also significantly improved OS.
